Objective1.To explore the childhood maltreatment of first-episode depression patients.2.To explore the influencing factors of cognitive function in patients with first-episode depression.3.To explore the relationship between cognitive impairment and childhood maltreatment in first-episode depression patients.MethodsNinety two patients with first-episode depression and ninety healthy controls were enrolled.The patients were divided into two group according to the the Childhood Trauma Questionnaire(CTQ-SF)score:64 patients with childhood maltreatment and 28 patients without.Hamilton Depression Rating Scale(HAMD-17)was used to assess the degree of depression.The Repeatable Battery For The Assessment Of Neuropsychological status(RBANS),Stroop Color Word Test(SCWT),Continuous Performance Test-Identical Pair(CPT-IP)were used to assess cognitive function in all subjects,and the Childhood Trauma Questionnaire(CTQ-SF)was used to assess whether there was childhood maltreatment(when any of the five major clinical items in the CTQ reached the cut-off value in this study,it is considered to be accompanied by childhood maltreatment).Results1.The total scores of childhood maltreatment,emotional abuse,physical abuse,sexual abuse,emotional neglect,and physical neglect in the depression group were higher than those in the healthy control group(P<0.05).2.Cognitive function test results showed that the immediate memory,attention,time-delay memory and RBANS total scores of the depression group were lower than the healthy control group(P<0.05).Stroop single word time,monochrome time,double word time,two-color error number,and two-color time were higher than healthy control group(P<0.05).There was no significant difference in CPT results between the two groups(P>0.05).3.Multiple linear regression results of depression group showed that multiple cognition such as immediate memory(β=0.511),speech function(β=0.337),and attention(β=0.405)were positively correlated with years of education;immediate memory(β=-0.203),time-delay memory(β=-0.205)was negatively correlated with age;CPT mean(β=-0.206)and other tests were negatively correlated with HAMD-17 total score.4.Multiple linear regression results with childhood maltreatment in depression group showed that there was a negative correlation between emotional neglect and speech function(β=-0.309);a positive correlation between emotional abuse and Stroop double word error number(β=0.865),Stroop double word time(β=0.781);a positive correlation between sexual abuse and Stroop two-color error number(β=0.250)and Stroop two-color time(β=0.270);CTQ total score and Stroop double-word error number(β=1.248)were positively correlated with Stroop double word time(β=0.184).Conclusion1.The first-episode depression patients have more serious childhood maltreatment than the normal population.2.The prevalence of cognitive impairment in patients with first-episode depression is mainly reflected in attention,memory and executive function.3.Age,years of schooling,and total HAMD scores have an impact on cognitive function.The younger the age,the longer the length of education,and the lower the total score of HAMD,the better the cognitive function of the subjects.4.Childhood maltreatment experiences have a negative impact on cognitive function.Emotional neglect can impair speech function,and emotional abuse and sexual abuse can impair executive function.
