Objective:To evaluate liver reserve function in patients with liver diseases using indocyanine green(ICG)clearance test,and so as to explore the correlation between ICG clearance test with Child-Pugh classification and the model for end-stage liver disease(MELD)score.Methods:A total of 103 patients with hepatic cirrhosis and 24 patients with hepatic failure admitted to the Department of Infectious Diseases,the First Hospital of Shanxi Medical University from August 2017 to August 2018 were enrolled,and all patients underwent ICG clearance test.Clinical test results and imaging examinations of all patients were collected,including ICG retention at 15 minutes(ICG R15),ICG plasma clearance rate(K value),effective hepatic blood flow(EHBF),abdominal color Doppler ultrasound,abdominal CT or nuclear magnetic,liver function,kidney function,blood coagulation test and blood cell analysis.According to the related examination results of the patients,MELD score and Child-Pugh score were calculated.And the hepatic cirrhosis was classified into three subgroups according to Child–Pugh classification : Child-Pugh grade A(n=41 cases),Child-Pugh grade B(n=42cases),Child-Pugh grade C(n=20 cases).The ICG R15,K value,EHBF and MELD score of each group of patients were compared.Analysis of the correlation between ICG R15 and biochemical indicators,K value and EHBF.Further analysis of the correlation between ICG R15 and Child–Pugh classification and MELD score.Results:The MELD score of the each group was statistically significant(P<0.05);Child–Pugh A,B group ICG R15 were lower than Child–Pugh C group and liver failure group,and Child–Pugh A group was lower than Child–Pugh B group,Child–Pugh A,B group K value and EHBF were higher than Child–Pugh C group and liver failure group,and Child–Pugh A group was higher than Child–Pugh B group,the differences were statistically significant(P<0.05);Spearman correlation analysis results show that ICG Rl5 was positively correlated with AST,TBil,and INR(r=0.338,0.727,0.590,P<0.05),while negatively correlated with K value,EHBF,PTA,Alb(r=-0.889,-0.784,-0.621,-0.569,P<0.05);There was a significant positive correlation between ICG R15 and Child–Pugh classification and MELD score(r=0.669,0.765,P<0.05).Conclusion:1.ICG Rl5 was positively correlated with AST,TBil,and INR,while negatively correlated with K value,EHBF,PTA and Alb.ICG R15 was affected by the above indicators of patients.2.ICG Rl5 has a good correlation with Child–Pugh classification and MELD score,and combining with each other can better reflect liver reserve function.
