Objective:To evaluate the diagnostic value of high-resolution CT in Hiatal hernia(HH).Methods:A retrospective analysis of the chest CT imaging findings of patients who underwent surgery for the diagnosis of esophageal hiatus hernia from Tianjin Medical University General Hospital and Tianjin Medical University Airport Hospital from January 2018 to January 2019 was performed,which were compared with the CT images of healthy examiners.All images were performed on the GEAW4.6 workstation.The quantitative indicators of the CT signs in patients with hiatal hernia were analyzed and evaluated,including esophageal advancement distance,gas volume in the esophagus,esophageal hiatus width,the His angle,abdominal esophageal length,fat thickness and qualitative indicators were sex and fat signs.The quantitative indicators were tested by the rank sum test or the t test according to the normal state.And the qualitative indicators were tested by c2.Statistically significant indicators were included in the binary logistic regression model.P values <0.05 were considered as statistical differences.Results:A total of 72 patients with esophageal hiatal hernia were enrolled.The patients with lacking clinical data and experiencing upper gastrointestinal surgery were excluded and 63 patients met the criteria.There were 52 healthy subjects in the control group.There was no significant difference in the age and sex ratio between the two groups(P>0.05).1.There was a statistical difference between the gas volume in the esophagus,the width of the esophageal hiatus,the length of the esophagus,the posterior esophageal wall,the fat thickness and the His angle between the case group and the normal group(P<0.05).There was no significant difference in esophageal advancement distance between the two groups(P>0.05).2.There was a statistically significant difference in the signs of fat between the case group and the normal group(P<0.05).3.Binary logistic regression showed that the increase of gas size,the His angle and posterior wall value in esophagus were risk factors for esophageal hiatal hernia.The hazard ratios were 1.40,1.08,and 1.62 respectively,and the 95% confidence interval was(1.14,1.73),(1.01,1.15),(1.08,2.42)respectively.The increase of esophageal length in the abdominal segment is a protective factor for the disease.The risk ratio is 0.63 and the 95% confidence interval is(0.49,0.79).Conclusion:By retrospectively comparing the high-resolution chest CT observations of the two groups of subjects,it can be seen that the gas in the esophagus,the width of the esophageal hiatus,the width of the posterior esophagus,the thickness of the fat,the increase of the His angle,and the presence of fat signs and esophageal hiatus in the lower esophageal sphincter.The occurrence of cockroaches is closely related.The increase in the length of the esophagus in the abdomen is a protective factor for the disease.
