| Objective:To explore the effect of ESWL ureteral stent in the treatment of 2-3cm renal calculi with RIRS.Methods and materials:60 patients(44 males and 16 females)with unilateral 2-3cm renal calculi treated by RIRS combined with holmium laser lithotripsy from November 2017 to November2018 in Urolithiasis Treatment Center of Second Hospital of Tianjin Medical University was studied retrospectively.All patients was divided equally into Group A and Group B,which was placed in the ESWL ureteral stent and traditional double J stent separatively.Both groups meet our diagnostic,inclusion and exclusion criteriaThe operation time was recorded and hemoglobin,white blood cells and albumin on the day after surgery were detected.The urinary color ultrasound and KUB were performed on the first day and two weeks after surgery.The two groups of patients were enrolled in the ureter-related symptom questionnaire(USSQ)at the time of extubation two weeks after surgery,and continuous attention was paid to whether or not there was debris discharge during urination before extubation and the formation of the Ureteral stone street after extubation RESULTS:There were no statistical differences in the differences(p>0.05)in the oper ation time(51.10±4.20 vs 51.07±3.89,min),postoperative hemoglobin(133.80±9.23 vs 132.50±3.72,g/L),postoperative leukocytes(7.41±1.54 vs 7.79±1.01,10^9),Postoperative albumin(39.60±4.58vs38.93±3.04,g/L),residual stone at dis charge(10/20vs9/21),postoperative urinary symptoms score(28.40±4.56vs27.54±4.74),postoperative physical pain symptoms score(22.50±4.19vs21.30±3.51),and lo wer kidney calyx residual stones before extubation(13/11/6vs12/10/8)between Group A and Group B.At the time of extubation,apart from lower kidney c-al yx,there were 23 patients(76.7%)and 13 patients(43.3%)without obvious calculi,4 patients(13.3%)and 8 patients(26.7%)with calculi less than 4mm,and 3 patients(10%)and 9 patients(30%)with calculi greater than or equ-alto 4mm in Group A and Group B separatively.There were significant stati-sti cal differences between the two groups(p=0.029<0.05).Also there were signifi cant statistical differences(p=0.001<0.05)in residual calculi which was 2 c-ase s(6.7%)to 13 cases(43.3%)and stone street which was 0 to 10 cases(33.3%).During the two weeks with stent,there were 11 cases(36.7%)and 3 cas-e s(10%)of stone excretion in Group A and Group B separatively,Alsothere wa s a significant statistical difference(p=0.015<0.05).Conclusion:Non-lower calyx residual stone,ureteral residual stone,formation of stone street after extubation and self-discharge of stones during catheterization in the patients with ESWL ureteral stent were significantly better than those with traditional stent.It is concluded that indwelling ESWL ureteral stent after RIRS for 2-3 cm renal calculi can significantly improve the effect of stone removal and improve the quality of life of patients,but still need to be further verified by big data. |
