Background Peri-implantitis is a common complication after implant repair and the main cause of implant failure.It is an infectious disease of peri-implant tissue which has formed bone bonding and function.The clinical manifestations are soft tissue infection around implants,bone resorption,bag formation around implants,accompanied by exploratory bleeding,sometimes spontaneous bleeding and pyorrhea.The goal of treatment is to control infection,eliminate peri-implant pockets,interrupt bone resorption and guide bone regeneration.The treatment methods include non-surgical treatment and surgical treatment.Water laser is a new therapeutic instrument,which has achieved positive results in the treatment of periodontitis.At present,water laser treatment of peri-implant inflammation is mostly limited to in vitro experiments,but relatively few clinical studies.In this experiment,water laser was introduced into the treatment of peri-implant inflammation to study its short-term clinical effect.Objective To evaluate the short-term clinical effect of water laser-assisted surgery in the treatment of peri-implant inflammation,and to provide a basis for the treatment of peri-implant inflammation.Method Fifty-two patients with peri-implant inflammation(60 implants)were collected from the Department of Stomatology,Hulunbeir People’s Hospital.The type of bone defect was intraosseous.The patients were randomly divided into two groups(30 implants in each group): experimental group and control group.The experimental group: non-surgical treatment + surgical treatment + water laser treatment;the control group: non-surgical treatment + surgical treatment.Modified plaque index(MPLI),periimplant bag depth(PPD),improved gingival sulcus bleeding index(MBI),diagnostic bleeding(BOP),marginal bone loss(MBL)around implants on apical radiographs were examined and recorded before treatment,6 months and 12 months after treatment.Thechanges of clinical indicators and imaging before and after treatment were compared between the two groups.Result During the whole course of treatment and after treatment,no discomfort occurred in all patients except for slight swelling in some patients after operation.There was no significant difference between the two groups in clinical indicators and imaging examination before treatment(P > 0.05),which was comparable.The measured values of improved plaque index(m PLI),peri-implant bag depth(PPD),improved gingival sulcus bleeding index(m SBI),probing bleeding(BOP)and marginal bone loss(MBL)around the implant were all lower than those before treatment(P < 0.05).After treatment,the measured values of improved plaque index(m PLI),peri-implant bag depth(PPD),improved gingival sulcus bleeding index(m SBI),exploratory bleeding(BOP),and marginal bone loss(MBL)around the implant were better in the experimental group than in the control group(P < 0.05).Conclusion Water laser assisted surgery for peri-implant inflammation can better reduce the inflammation of peri-implant tissue,eliminate the bleeding,reduce the depth of peri-implant bag,repair bone regeneration in the short term.It has better clinical efficacy than traditional treatment methods.It shows that water laser-assisted surgery for peri-implant inflammation has good application prospects.
