| Objective:To analyse the clinical efficacy of different surgical methods in patients with pelvic abscess and to explore the best surgical method and surgical timing in patients with pelvic abscess.Methods: Retrospective analysis of the 127 patients who were operated on for pelvic abscess in Affiliated hospital of Qingdao university from January 2012 to December2017,postoperative pathology confirmed pelvic abscess.Patient were divided into laparoscopic surgery group and laparotomiy group according to different surgical methods.The preoperative antibiotic application time within 3 days was divided into two groups : A1(laparoscopic group,54 patients were included),B1(open group,21 patients were included).The preoperative antibiotic application time over 3 days was divided into two groups :A2(laparoscopic group,30 patients were included)and B2(open group,21 patients were included).The differences in patients’ age,pelvic mass size,clinical symptoms,intraoperative blood loss,operation time,postoperative bowel function recovery time,postoperative antibiotic application time and postoperative hospitalization days were compared,and the postoperative recovery of the patients was generally evaluated.So as to select the best surgical method for patients with pelvic abscess.To determine the type of pathogen infection,all patients were extracted from pus for secretion culture during the operation.Also,patients,whose temperature exceeding 38.5degrees,were collected blood culture for examination.Clinical staff could use different antibiotic treatment according to the different pathogen.In this paper,all the data were analyzed using SPSS 17.0 software,and P < 0.05 was considered statistically significant.Results:The group A1 were significantly better than those in group A2 from intraoperative blood loss,operative time and hospitalization days after operation,and the difference was statistically significant(P<0.05).The group A1 were significantly better than those in group B1 from intraoperative blood loss,operative time,postoperative recovery time of intestinal and hospitalization days after operation,and the difference was statistically significant(P<0.05).The group A2 were significantly better than those in group B2 from intraoperative blood loss,operative time,hospitalization days afteroperation and postoperative recovery time of intestinal,and the difference was statistically significant(P<0.05).However,there were no significant differences in intraoperative blood loss,operative time,hospitalization days after operation and postoperative intestinal function recovery time between group B1 and group B2(P>0.05).Conclusion:1.The main method to cure pelvic abscess is the combination of surgical treatment and drug treatment.2.Once patients with pelvic abscess are found,they should actively carry out anti-inflammatory treatment and prepare for surgery.3.Laparoscopic surgery is the first choice for pelvic abscess patients.For those patients with severe pelvic adhesions who can not continue laparoscopic surgery,conversion to open surgery may be considered. |
