| Objectives To explore the prognostic effect of the simple assessment scale for patients with acute cerebral infarction in anterior and posterior circulation,and to evaluate whether the simple assessment scale is more suitable for patients with anterior or posterior circulation cerebral infarction,so as to provide reference for its accurate clinical application.Methods Patients with acute cerebral infarction were selected from the Department of Neurology,Affiliated Hospital of North China University of Technology from January2015 to September 2018.The patients diagnosed as anterior circulation acute cerebral infarction were in the anterior circulation group,and the patients with posterior circulation acute cerebral infarction were in the posterior circulation group.According to the diagnostic criteria,inclusion criteria and exclusion criteria,1432 patients were enrolled,including 973 in the anterior circulation group and 459 in the posterior circulation group.Within 24 hours of admission,the selected patients were collected with current medical history,past history,physical examination and auxiliary examination results,and scored with the Simple Assessment Scale.The prognosis of one month after onset was taken as the end point.All patients were followed up.The outcome was evaluated by mRS score.The prognosis was good when the mRS score < 2 was defined as one month after onset,and bad when the mRS score > 2 was defined as one month after onset.Quantitative data were analyzed by t-test,counting data and classified variables were analyzed by 2 test,and the working characteristic curve(ROC) of subjects with simple assessment scale was drawn and the area under the curve(AUC)was calculated.The optimal threshold value was obtained by maximum Yoden index,and the Kappa value was calculated according to the actual clinical situation.The effect of simple assessment scale on patients with acute cerebral infarction in anterior and posterior circulation was discussed.The predictive value of one-month prognosis.Results 1 There was significant difference in the score of the simple assessment scale between the anterior circulation group and the posterior circulation group(P<0.05),and the value of the anterior circulation group was lower than that of the posterior circulation group.2 The poor prognosis rate of high score patients in anterior circulation group was higher than that of low score patients,with statistical significance(P<0.05);the poor prognosis rate of high score patients in posterior circulation group was higher than that of low score patients,with statistical significance(P<0.05);with the increase of the score of simple assessment scale,the poor prognosis rate of anterior and posterior circulation group showed an upward trend.3 The area under the ROC curve was 0.846,the sensitivity was80.4%,the specificity was 73.2%,and the diagnostic threshold was 4.5.The area under the ROC curve was 0.794,the sensitivity was 78.7%,the specificity was 69.4%,and the diagnostic threshold was 4.5.4 The overall consistency of prognosis was 0.756 with Kappa value 0.497 in the anterior circulation group and 0.747 with Kappa value 0.482 in the posterior circulation group.Conclusions 1 Simple Assessment Scale has reference value in predicting the prognosis of patients with acute cerebral infarction for one month.2 Simple Assessment Scale has a higher predictive value for one month prognosis of patients with anterior circulation acute cerebral infarction than that of patients with posterior circulation acute cerebral infarction.Figure3;Table15;Reference 103... |
