Objective:This study conducted a community randomized controlled trial to evaluate the effect of O2O health management service process of hypertension in young and middle-aged people,and put forward optimization suggestions,so as to improve the implementation of O2O hypertension health management of young and middle-aged people in the community.Methods:Using the randomized controlled trial method,220 young and middle-aged people aged 19 to 60 were selected from community health service centers of Mishixiang,Hangzhou,and were randomly assigned to two groups.There was no statistically significant difference in gender,age,condition of hypertension between two groups.After implementing the O2O health management service process and the regular health management service process intervention for half a year,the effect is evaluated through hypertension control level,hypertension knowledge,management compliance,further management needs and satisfaction.(?)±S was used to represent the measurement data,paired t test was used to compare the differences before and after management,and group t test was used to compare the differences between groups.Counting data was expressed by rate or composition ratio,the difference between two groups was compared by chi-square test,and the influencing factors of young and middle-aged patients' satisfaction about hypertension health service process were analyzed by binary logistic regression.P<0.05 was considered statistically significant.Results:1.Hypertension control level: the mean systolic blood pressure before management was 153.20±15.19 mm Hg,the mean diastolic blood pressure was96.68±12.09 mm Hg,the mean systolic blood pressure after management was139.90±9.09 mm Hg,the mean diastolic blood pressure was 89.01±7.04 mm Hg,the difference between systolic blood pressure and diastolic blood pressure before and after management was statistically significant.However,there was no significant difference in systolic blood pressure(152.41±15.04 mm Hg vs.145.30±14.21 mm Hg)and diastolic blood pressure(96.47±11.55 mm Hg vs.93.28±7.34 mm Hg)before and after management in the control group.There were 5 cases(4.5%)of poor hypertension control in the intervention group and 21 cases(19.4%)in the control group,and the difference between two groups was statistically significant.2.Knowledge level and behavior change: Comparing with the control group,intervention group had higher hypertension knowledge scores(62.34±3.83 vs.30.72±3.09),higher rates of blood pressure measurement(69.1% vs.23.1%),smoke reduction(31.8% vs.15.7%),drunk reduction(38.2% vs.13.9%)and salt reduction(87.3% vs.38.0%)and physical exercise(81.8% vs.31.5%),more regular lifestyle(84.5% vs.29.6%)and better blood pressure control(90.1% vs.51.9%),the difference between two groups was statistically significant.3.Satisfaction analysis: The patients in the intervention group were significantly more satisfied with health management of hypertension than those in the control group(94.5% vs.67.6%),and the difference of demand was also statistically significant(P<0.01).According to single factor analysis,there existed statistical difference among the gender,age,hypertension drugs and blood pressure control(P<0.05).Multivariate analysis found that women(OR=1.61,P<0.01),50?60 years old(OR=1.41,P<0.05),less than one type of hypertension drug(OR=1.71,P<0.01),stable blood pressure control(OR=2.23,P<0.05)were more satisfied with the O2O health management service process.4.Further demands for service process: 192 patients(88.1%)expected to receive health management from hospitals or doctors,178(81.7%)expected We Chat/QQ,165(75.7%)expected mobile APPs,and 45(20.6%)expected community management.The contents of health management that patients hoped to obtain include food guidance(90.8%),lifestyle guidance(88.0%),psychological guidance(77.9%),sports guidance(74.0%)and disease knowledge(73.1%).Conclusion:(1)Compared with regular health management service process,the O2O health management service process of hypertension for young and middle-aged people has more effective control,lower adverse incidence,improved hypertension knowledge,management compliance and satisfaction.(2)The O2O hypertension health management process in young and middle-aged people can realize remote and real-time management of patients,improve the management effect,and effectively improve the quality and speed of service.The application of this method will play an important role in improving the efficiency of hypertension health management in young and middle-aged people.(3)The satisfaction of O2O health management process is influenced by the gender,age,hypertension medication types,and blood pressure control of young and middle-aged hypertension patients.Females,50?60 years old,less than one type hypertension drug,stable blood pressure control have higher satisfaction with O2O health management service process.(4)In order to promote O2O health management service process of hypertension among young and middle-aged people,firstly the design should conform with positioning of community health service agencies and enhance the accessibility and usefulness,and also cater to the preferences of young and middle-aged people.Secondly,community health service centers should actively promote the adoption of O2O hypertension health management service process.Finally,young and middle-aged patients with hypertension should make full use of online and offline approaches to hypertension health management.
