Objective : To understand the incidence and influencing factors of sleep disorders in elderly patients with chronic diseases,and to establish a risk prediction model for sleep disorders in elderly patients with chronic diseases,which lays a foundation for early identification of high-risk individuals and timely intervention,reducing hospitalization rate and improving their quality of life.Methods:328 elderly patients with chronic diseases were enrolled in the Department of Geriatrics,the First Affiliated Hospital of Chongqing Medical University.The basic information,basic data,life behavior habits,medical history,etc.of the subjects were collected using the comprehensive evaluation of the elderly,and the patient's sleep and daily routine were collected.Assessment of life ability,pressure sore risk,nutritional status,swallowing function,cognitive function,mental health,etc.SPSS 23.0software was used for statistical analysis.The influencing factors of sleep disorders in elderly patients with chronic diseases were determined bysingle factor and multi-factor analysis.The risk prediction model was established and the diagnostic value of the constructed model was evaluated by ROC curve.Results : The average score of PSQI in 328 elderly patients with chronic disease was 10.165±4.0626,and 205 elderly people with sleep disorders accounted for 62.5%.In the single factor analysis,women in the sleep disorder group,living alone,never exercise,irregular life,visual impairment The prevalence of urinary incontinence,anxiety,depression,type 2 diabetes,chronic gastritis was significantly different from that of non-sleep disorder group(P<0.05).Multivariate logistic regression analysis showed that living alone,visual impairment,and urinary incontinence,anxiety,depression,diabetes,chronic gastritis are risk factors for sleep disorders in elderly patients with chronic disease;according to Logistic regression analysis results to establish a model: Logit(P)=-1.546 + 0.682* living conditions + 0.890 * visual impairment + 1.223 * urinary incontinence + 0.823* anxiety +0.925*depression+0.688* type 2 diabetes+0.756* chronic gastritis,the area under the ROC curve of the model was0.791(P<0.01,95% CI: 0.740,0.843);7 obtained by logistic regression analysis The index establishes a predictive scoring system for sleep disorders in elderly patients with chronic diseases,with a total score of 8points.The area under the ROC curve is 0.786(P<0.01,95% CI: 0.735,0.838),with a margin of 3 The sensitivity was 75.6%,specificity 71.5%.Conclusion:Sleep disorders are high in the elderly chronic patient population.Women,lack of physical exercise,and irregular life are related to sleep disorders;living alone,visual impairment,urinary incontinence,anxiety,depression,type 2 diabetes,and chronic gastritis are independent risk factors for sleep disorders in elderly patients with chronic diseases.This study established a simple and predictable risk prediction model and scoring system for sleep disorders in elderly patients with chronic diseases.It has certain clinical applicability for early diagnosis and intervention of high-risk individuals,and it is worthwhile to further study on the basis of expanding the sample.
