Objective: To explore weather the spleen volume influence the short—term efficacy of splenectomy for the treatment of immune thrombocytopenia(ITP).Methods: Spleen volume index(SI)was calculated by maximal width(cm)× thickness(cm)× length(cm),67 ITP patients who underwent splenectomy were hospitalized in The First Affiliated Hospital of Chongqing Medical University from April 2013 to October 2017 were selected,those patients were divided into two groups: normal and splenomegaly according to the spleen volume.The number of platelets transfused before operation,accessory spleen,postoperation hospital stay length,the amount of bleeding during operation,the number of blood transfused during operation,complications,the platelet count at admission,the last platelet count before operation,the platelet count on the 7th day after operation,the platelet count on the 30 th day after operation and the short-term starting efficiency(30 day efficiency)were compared between the two groups.Results: 28 cases with normal spleen volume,and the other 39 cases with splenomegaly,there was no significant difference in general data of gender,age and preoperative course between the two groups(P>0.05).There were no significant differences in the number of platelets transfused before operation,accessory spleen,postoperation hospital stay length,length of operation,amount of bleeding during operation,number of blood transfused during operation,complications,platelet count at admission,last platelet count before operation,platelet count on the 7th day after operation and platelet count on the 30 th day after operation between the two groups(P>0.05).The short-term initial efficiency of the splenomegaly group was 76.92% higher than that of the normal group(67.16%),and the difference was statistically significant(P < 0.05).Conclusions: 1.Splenectomy is a safe and effective treatment of ITP.2.Splenic volume is an indicator that can effectively predict the short-term efficacy of splenectomy in ITP patients.
