Clinical Application Of Mitomycin,Amniotic Membrane And Stromal Lens In Trabeculectomy

Objective:To investigate the etiology of corneal endothelitis,To study the value and significance of real-time fluorescence quantitative polymerase chain reaction(RQPCR) in the diagnosis of corneal endothelitis in terms of etiology.Method:Follow-up between September 2017 and August 2018 in our hospital treated by trabeculectomy combined mitomycin covering 22 cases(22 eyes),trabecular resection combined biological amniotic membrane covering 24 cases(24 eyes),trabecular nephrectomy combined corneal lens cover 13 cases(13 eyes) cases postoperatively in patients with primary open-angle glaucoma,statistical analysis of three groups of postoperative(1 month,3 months,6 months),intraocular pressure change,filtering bleb morphology under the slit lamp,the AS-OCT filtering bleb points.Results:After trabeculectomy,the mean intraocular pressure at 1 month was 12.15 mmhg in mitomycin group < 12.35 mmhg in amniotic membrane group < 15.73 mmhg in lens group.Mean iop at 3 months: 12.48 mmhg in mitomycin group < 15.03 mmhg in amniotic membrane group < 18.01 mmhg in lens group;Mean iop at 6 months: 13.38 mmHg in mitomycin group < 15.73 mmHg in amnion group < 18.61 mmHg in lens group,and lower iop in mitomycin group.At 1,3 and 6 months after surgery,it was proved that the formation rate of functional filtration vesicle under slit lamp and the formation rate of diffusion type + microvesicle type of filtration vesicle detected by AS-OCT were both better in mitomycin group and amniotic membrane group than in lens group,and there was no difference between mitomycin group and amniotic membrane group.Conclusions:Our research suggests that for patients with primary open-angle glaucoma,application of mitomycin in trabeculectomy,biological amniotic membrane,corneal stroma lens,mitomycin,biological amniotic membrane can form functional filtering bleb,but for the target intraocular pressure in patients with lower mitomycin is the best choice,application of corneal stroma after lens functional filtering bleb formation is not ideal.Therefore,the application of mitomycin in trabeculectomy is the first choice for patients with primary glaucoma.
