Objective:To explore the relationship between different ages and the lumbar facet joint orientation in both the axial and sagittal planes,and the correlation between facet joint oriention with facet joint tropism and female degenerative lumbar spondylolisthesis。Methods:1.A total of 300 patients(without lumhar disease)for whom abdominal and pelvic computed tomography scans were obtained at our hospital between January 2017 and December 2012 were included.Patients were divided into five groups according to age(≤35,36~45,46~55,56~65,≥ 66 years),30 males and 30 s in each group.A three-dimensional reconstruction image of the lumbar spine CT was obtained,and the axialand sagittal joint angles of the five segments of L1-S1 were measured.The relationship between age and joint angle was discussed.。2.A total of 73 patients with anterior spondylolisthesis who were admitted to our hospital from January to February 2017 and inpatient hospitals were enrolled in strict accordance with the inclusion and exclusion criteria.The average age of the DS group was58.27 ± 7.50 years(range 46-76 years),54 females and 19 males.In the patients with abdominal and pelvic CT thin-slice scan without significant waist and leg pain,73 patients with gender and age matched were selected as the control group.The joint angles were measured in the axial and sagittal positions,and the left and right lateral differences were calculated.Values,defining joint asymmetry of 7°,were used to count the incidence of joint asymmetry in both groups.The SPSS 24.0 software package was used to compare the two-sample mean and the incidence of joint asymmetry.Results:1.L1-2 and L2-3,the axial joint angle of male and female and the sagittal joint angle of male were not significantly different between different age groups(P>0.05);2.After the age of 46,with the increase of age,the axial joint angle decreases and the sagittal joint angle increases.The axial joint angle is mainly expressed in L3-4 in males and L4-5 and L5-S1 in females;sagittal joint angles are mainly expressed in L4-5 and L5-S1 in males and mainly in L4-5 in females.3.In female L4 spondylolisthesis,the joint angle of L4-5 level,axial position and sagittal position was statistically different between DS group and control group.The former was more sagittal and horizontal than the latter(P<0.05).There was a statistically significant difference in the sagittal joint angle between the DS group and the control group(P<0.05),and there was no significant difference in the axial joint angle(P>0.05).There was no significant difference in the joint angle between the two groups at the L5-S1 level,axial position and sagittal position(P>0.05).4.The number of axial joint asymmetry was statistically different between the DS group and the control group at the L4-5 level(P<0.05).The former had more axons with axial asymmetry,but at adjacent levels L3-4 and L5-S1.There was no significant difference in joint asymmetry between the two groups(P>0.05).There was no statistical difference in the number of sagittal joint angle asymmetry between L3-4,L4-5 and L5-S1(P>0.05)..Conclusion:1.With the increase of age,the lower lumbar facet joints gradually decrease in the axial position,and gradually increase in the sagittal position,ie,sagittal and horizontal development.2.The L4-5 level of the facet joints in patients with degenerative lumbar spondylolisthesis showed more pronounced sagittal and horizontal,and the joints had more axillary asymmetry in the axial position.At the same time,the level of L3-4 on the cephalad is also more horizontal in the sagittal position.
