Clinical Value Of Transabdominal Preperiton Hernioplasty For Occult Inguinal Hernia In Adults

Objective:To study the effect of transabdominal preperiton hernioplasty(TAPP)on occult inguinal hernia in adults,and to explore the incidence of occult inguinal hernia in adults,as well as the value of age,sex,BMI,hernia location and size of hernia ring in predicting the occurrence of occult inguinal hernia in adults.Methods:Retrospectively collect the data of patients with inguinal hernia in general surgery department of the Second Hospital of Shanxi Medical University from September 2016 to October 2018,and select the cases treated with TAPP as the research object.According to the results of TAPP peritoneal exploration,the patients diagnosed as unilateral inguinal hernia before operation were divided into two groups.Group A was diagnosed as occult contralateral inguinal hernias(OCH)by endoscopy,and group B was negative by endoscopy.The age,sex,BMI,hernia location,size of hernia ring,duration of illness,length of hospitalization,duration of operation and operation cost were compared between the two groups.Results:A total of 129 patients were enrolled in the study.18 cases of occult inguinal hernia(14%)were found,with an average age of 65 years,12 cases on the right side,6cases on the left side,12 cases of indirect hernia,4 cases of direct hernia and 2 cases of saddle hernia.There were 18 cases in group A and 79 cases in group B.There were significant differences in the size of hernia rings,operation time and hospitalization expenses between the two groups(t=10.496、t=6.490、t=5.933;p<0.05).There were no significant differences in age,gender,hernia location,BMI,duration of illness and length of hospitalization(?~2=4.082、?~2=0.465、?~2=0.215、t=-0.921、t=0.909、t=-0.973;p=0.130、p=0.495、p=0.643、p=0.359、p=0.365、p=0.333).Conclusion:For adult patients with unilateral inguinal hernia,TAPP has the advantage of finding contralateral occult inguinal hernia and predicting the related factors of occult inguinal hernia.TAPP can effectively avoid the occurrence of secondary hospitalization and reoperation in suspicious cases.It is also recommended for the treatment of new hernia and recurrent anterior hernia.
