| Objective:To investigate the clinical significance of PLA2 R antibody and THSD7 A antibody titer in serum and urine of patients with membranous nephropathy in patients with idiopathic membranous nephropathy.Methods:Serum and urine PLA2 R antibodies and THSD7 A antibodies were determined by enzyme-linked immunosorbent assay(ELISA)in patients with membranous nephropathy confirmed by renal biopsy from July 2015 to December 2017 in the Department of Nephrology,Second Hospital of Shanxi Medical University.Concentration levels were analyzed and correlated with gender,age,24 h urine protein quantitation,serum albumin,serum creatinine,uric acid,urea nitrogen,glomerular filtration rate,cystatin C and other clinical indicators.Results:From July 2015 to December 2017,189 patients were diagnosed as MN clinically and pathologically(LM,IF and EM),among which 165 patients were in the IMN group(87.3%).(1)The positive rates of serum and urine PLA2 R antibodies in IMN patients were 64.8% and 60.0%,respectively,and the positive rates of serum and urine THSD7 Aantibodies were 8.4% and 5.4%,respectively.(2)serum and urine PLA2 R antibody positive and negative in IMN patients showed no statistical significance in gender,age and clinical indicators;(3)The serum and urine PLA2 R antibody concentration of IMN patients was positively correlated with the amount of urinary protein at 24 h,and negatively correlated with albumin.(4)serum and urine THSD7 A antibody concentrations in IMN patients were positively correlated with 24 h urine protein quantification,but not with albumin;(5)IMN,SMN and UAMN had no statistical significance in age,24 h urine protein quantification,albumin,total cholesterol.(6)Serum PLA2 R antibody concentration was positively correlated with urine PLA2 R antibody concentration in IMN patients,and serum THSD7 A antibody was positively correlated with urine THSD7 A antibody concentration.(7)test serum,urine PLA2 R antibody for diagnostic IMN sensitivity were 63.6%,99.0%,specificity were 100%,75.0%.The sensitivity of serum PLA2 R antibody positive,urine negative,serum negative,urine positive,serum and urine positive in the diagnosis of idiopathic membranous nephropathy was 78.7%.Conclusion:PLA2R antibody in serum and urine as well as THSD7 A antibody can be used as specific indicators for IMN diagnosis.Compared with THSD7 A antibody,PLA2 R antibody detection rate is higher,and the concentration level of these two antibodies in serum and urine has a certain correlation with 24 h urine protein quantification and serum albumin. |
