Purpose To investigate the relationship between eosinophil and the prognosis of acute ischemic stroke(AIS).Methods Laboratory investigptions, neuroimaging and modified Rankin Scale(mRS)score were collected from patients admitted to the Second Aiffliated Hospital of Soochow University from January 2017 to February 2018.All patients were divided into four group according to their level of eosinophil percentage.Receiver operating characteristic(ROC)analysis was used to conpare the accuracy of neutrophik and eosinophils in predicting large infarct volume. Moreover, the relationship between eosinophil percentage and infarct voluaes, infection rates and poor outcome(mRS>3)was examined by multivariate regression anafysis. Results Total 421 eligible patients were enrolled into this study.Binary logistic regression analysis revealed that eosinophil percentage was an independent risk factor for AIS(P=0.002).Significant correlation was found between eosinophil percentage and infarct voluhe(Spearman rs=-0.514,P<0,001).ROC analysis showed that eoshophils could predict the large infarct volume more accurately than neutrophils(their respective area under curve(AUC):0.906 and 0.876).Further multivariate analysis showed that higher infection rate, larger infarct volume and poor outcome remained strongly associated with tower eosinophil percentage(P-trend<0.001).Conclusion Low eosinophil has the potential to predict the poor outcome of AIS.
