Objective To compare the hemostatic effects and complications between two balloon catheter traction methods after transurethral resection of prostate(TURP)and provide a theoretical basis for seeking the optimal catheter traction method with better hemostatic effects and fewer complications.Methods This study included 80 patients who received TURP in Taixing people’s hospital from July 2017 to July 2018 according to special standard.AH the patients were immediately followed by traction with balloon urethral catheter after operations.Patients were divided into two groups according to the method of catheter traction,group A with groin traction method(40 cases)and group B with rubber band traction method(40 cases).Both groups of patients signed written informed consent after admission.The patients were all performed TURP by the same professor under combined spinal and epidural anesthesia.The type and speed of the intraoperative and postoperative flushing fluid were the same.Self-rating anxiety scale(SAS)was used to evaluate the emotional state of the two group patients.Bladder spasm symptom scale(BSSS)was used to quantify the degrees of bladder spasm.The average erythrocyte counts of the two groups of patients were measured and recorded at 4 time periods(within 6h after surgery,within 6h~12h after surgery,within 12h~18h after surgery,and within 18h~24h after surgery).The incidence of relevant complications(urethral external compression injury,lower extremity deep venous thrombosis,pressure ulcer/red)was compared.The time of operation,the weight of the resected prostate,the total amount of bladder flushing fluid,the time of the conversion of bladder flushing fluid and the number of days of hospitalization were reeorded.Finally,the differences between the data of the two groups were compared and related statistical analysis was conducted.Results The degree of postoperative anxiety and bladder spasm of rubber band traction method were better than that of inguinal region method.The erythrocyte counts of bladder drainage fluid decreased in two groups gradually over time,and the difference was significant(P<0.05).The erythrocyte counts of the flushing fluid in rubber band traction method were significantly lower than that in the inguinal region in 4 time periods within 24h after operation(P<0.05).The total amount of bladder flushing fluid and the number of days of postoperative hospitalization in rubber band traction method were less than that of the inguinal region method(P<0.05).The time of bladder flushing fluid converting limpidity after operation in rubber band traction method was shorter than that of inguinal region method.There was statistically significant difference in complications of compression injuries of the external urethral orifice(P<0.05).There was no significant difference between the two groups in lower extremity deep venous thrombosis and pressure ulcer/red(P<0.05).Conclusions In this study,it is shown that the traction of rubber band by urethral catheter method has a high degree of comfort,and the complications are less and the postoperative bleeding 24 hours after TURP is effectively reduced.The results of this study indicate that application of rubber band catheter traction after TURP can reduce the total amount of bladder flushing fluid and shorten the length of hospitalization of patients,which is worthy of application and promotion in TURP. |
