Objective:This study was aimed to investigate the value of 99mTc-MIBI scintigraphy assessment of neoadjuvant chemotherapy for osteosarcoma.Methods:One hundred and two patients with osteosarcoma underwent 99mTc-MIBI dual-phase imaging before neoadjuvant chemotherapy and after two cycles of chemotherapy,respectively.GE Infinia VC Hawkeye 4 SPECT/CT was used with parallel hole low energy high resolution collimator,140keV energy peak,20%window width.After intravenous injection of 925MBq(25mCi)tracer,99mTc-MIBI imaging was performed with early phase and delayed phase at 5-10min and 120min,respectively.Anterior and posterior whole body imaging as well as local imaging of lesion areas and control sites were performed with the same acquisition conditions for each scintigraphy.The imaging agent--99mTc-MIBI was provided by the Chinese Atomic Energy Research Institute.Qualitative and semi-quantitative analysis was carried out.Uptake ratio(UR)and alteration of uptake ratio(AUR)are calculated by measuring the radioactive of tumor lesions and normal tissues.SPSS 17.0 software was used for data analysis.Results:(1)One hundred and two cases with osteosarcoma were involved in this study.According to tumor necrosis rate(TNR),102 patients was classified into three(2)group:30 cases in group 1(TNK less than 50%),52 cases in group 2(TNR:50%-89%)and 20 cases in group 3(TNR greater than 90%).The features such as uptake ratio(UR)were obtained in early images before and after chemotherapy.The alteration of uptake ratio(AUR)also were calculated and according to tumor necrosis ratio(TNR).(3)In 99mTc-MIBI images,URpre and URpost of group 1 were 1.68±0.44 and 1.83±0.71 respectively,the changes of UR had no statistical significance(P>0.05);URpre and URpost of group 2 were 2.42±1.42 and 1.59±0.71 respectively,the changes of UR had statistical significance obviously(P<0.05);URpre and URpost of group 3 were 2.58±0.61 and 1.21±0.16 respectively,the changes of UR had statistical significance obviously(P<0.01);(4)The AUR value in 99mTc-MIBI scintigraphy was-0.97?0.64,the liner regression analysis of AUR versus TNR showed a highly significant positive correlation(r=0.76,P=0.02).Conclusions:The AUR changed significantly after the neoadjuvant chemotherapy and had a highly significant positive correlation to TNR.It is significant that 99mTc-MIBI scintigraphy to be used in the evaluation of ncoadjuvant chemotherapy.
