Objective:To evaluate the TCM constitution types of patients with age-related macular degeneratio with drusen and probes into the characteristics of TCM physique in the further development of early AMD.T o provide clinical evidence for early prevention and treatment of age-related macular degeneration.Methods:A total of 180 patients who met the inclusion criteria at the Ophthalmology Hospital of China Academy of Chinese Medical Sciences from May 23,2017 to February 1,2019,registered general information,color fundus photography,optical coherence tomography(OCT)After the examination,the visual acuity test and the Amsler test were grouped into small drusen group,medium drusen group,large drusen group,and geographic atrophy group,CNV group;fill in the "Chinese medicine body quality table" to test the type of TCM constitution;use Excel 2018 software to enter the data of the collected patients into the computer,review the patient data,check and form the original database.Statistical analysis was performed using SPSS 22.0 statistical software.Ordered multi-class logistic regression tests were performed on the mall drusen group-geographic atrophy group and mall drusen group-CNV group,respectively.The measurement data was first tested for normality.If the normal distribution was followed,the T test was used.If the normal distribution was not followed,the non-parametric test was used.The data were analyzed by x2 test.P≤0.05 was considered statistically significant.Result:1.All types of dry AMD drusen patients physical classification statistics:dry AMD drusen patients with TCM constitution type characteristics of yin deficiency is the most significant,a total of 52 cases;followed by qi deficiency There were 43 cases;32 cases of yang deficiency;the other types of body type distribution were not much different,including 18 cases of phlegm dampness,12 cases of damp heat,13 cases of blood stasis,11 cases of special sputum,17 cases of qi stagnation,and at least 8 cases of peace2.Ordered multi-class logistic regression test results:Yin deficiency and qi deficiency are related to the development of Drusen to geographic atrophy(P1=0.025,P2=0.035),and the regression coefficients are 0.812 and 0.743.respectively,or e0.0812=2.25,and e0.743=2.10.Humidity,damp heat,blood stasis and qi stagnation were related to the development of CNV(both P<0.05).The regression coefficients were 1.345,0.991,1.918 and 1.304.respectively,and the OR values were e1.345=3.84,e0.991=2.69,e1.918=6.81 and e1.304=3.68.3.The distribution of TCM constitution types among patients with different degrees of drusen:the proportion of Yin deficiency in the patients with only drusen and geographic atrophy in the fundus is the highest.Small Drusen group-large Drusen group account respectively.For:33%,33%,50%,61%;followed by qi deficiency,Small、medium、large Drusen group、Drusen to geographic atrophy group accounted for:25%,31%,42%,47%;yang deficiency Group,Smal、medium、large Drusen group、Drusen to geographic atrophy group accounted for:19%,22%,25%.and 41%,respectively;others were irregular;the number of peacetime samples decreased gradually.Small、medium、large Drusen group、Drusen to geographic atrophy group the proportion of d group was:28%,17%,5%,0%.In addition to the sputum,the number of all kinds of biased constitutions in the CNV group was not much different,and there was no peace.4.X2 test results:the distribution of yin deficiency was statistically significant among the groups(P=0.047),and the proportion of yin deficiency in the map-like atrophy group was 61%.The distribution of phlegm-dampness,blood stasis and damp heat in each group was statistically significant(P<0.05),and the three in the CNV group accounted for the highest proportion:phlegm-dampness 36%,blood sputum 39%,damp-heat mass 33%.There is a difference in the distribution of peace between groups.(P<0.05),and the proportion of the peace of the small drusen was 28%.5.The age statistics of the patients in each group were significantly different between the small drusen group and the middle drusen,the large drusen,and the geographic atrophy group(P<0.05).The difference between the large drusen group and the CNV group was statistically significant.(P<0.05.)There was no significant difference between the large drusen group and the middle drusen group(P>0.05).The average age of each group was 66 years old in the small drusen group,71 years old in the middle drusen group,74 large drusen group,73 years in the geographic atrophy group,and the CNV group is 68 years old.6.Statistical results of TCM constitution type classification:The gender difference in qi stagnation was statistically significant(P=0.05),and the number of qi stagnation in female patients was 23%;the gender difference in peace and quality was statistically significant(P<0.05),the proportion of male patients with high quality was 12%.7.Visual function:After T test,the difference of visual acuity between different groups was not statistically significant(P>0.05).The results of Amsler test for different degrees of Drusen patients were tested by X2 test.The Amsler test in different groups did not have statistics.Significance(P>0.05).Conclusion:1.The occurrence of Drusen is closely related to the yin deficiency,qi deficiency and yang deficiency.2.The probability of developing yin deficiency and qi deficiency in patients with geographic atrophy is greater;patients with phlegm-dampness,damp-heat quality,blood stasis and qi-stagnation have a higher probability of developing bleeding.As the age increases,the drusen increases and AMD progresses. |
