Objective To compare the differences of blood and 24-hour urine metabolic indexes between children with calcium oxalate calculi(case group)and children without stones(control group),and to explore the metabolic characteristics and appropriate metabolic assessment methods in children with calcium oxalate stones.To provide a basis for the prevention and treatment of urinary calculi in children.Methods A case-control study.From August 2016 to October 2018,75 patients who were hospitalized and whose main component was calcium oxalate were selected as the case group and the same year of birth as the case group.Blood samples and 24-hour urine samples were collected from 75 non-stone children of the same sex in our hospital as control group.The results of blood test(potassium,sodium,chlorine,calcium,creatinine)were compared between the case group and the control group.Urea nitrogen and uric acid concentrations)and 24-hour urine test results(urine volume,urine calcium concentration,calcium metabolism per body weight,oxalic acid concentration,oxalic acid excretion per unit surfacearea,citric acid concentration,citric acid unit surface area excretion),Phosphorus concentration).Results 1.There was no significant difference in age,height and weight between the two groups(P > 0.05).2.There was no significant difference in total calcium per unit weight(P >0.05).The citrate per unit surface area in the case group was lower than that in the control group,the difference was statistically significant(P <0.05).3.24-hour total urine volume and urine concentration of related metabolites: total urine volume,total calcium concentration,total urine volume,total calcium concentration in the two groups.There was no significant difference in phosphorus concentration(P > 0.05).The urinary oxalic acid concentration in the case group was significantly higher than that in the control group(P < 0.05),and the urinary citric acid concentration in the case group was lower than that in the control group(P < 0.05).4.There was no significant difference in serum potassium,calcium,chlorine,urea nitrogen,creatinine,uric acid between the two groups(P >0.05).The serum sodium in the case group was significantly higher than that in the control group(P < 0.05).Conclusion1.The concentrations of oxalic acid and citric acid in urine of childrenwith calcium oxalate stones were significantly different from those of children without stones.There was only a difference in the excretion of citric acid per unit surface area between the two groups.2.There was little difference between calcium oxalate stone children and non-stone children in the excretion of related metabolites,but the difference was more significant in calcium oxalate stone children than in non-stone children.Urinary concentrations of metabolic substances may be more accurate in assessing the risk of calcium oxalate gallstone formation in children.3.The serum sodium concentration in children with calcium oxalate stones was higher than that in children without stones.
