Research objectiveWe used press-needle combined with canalith repositioning maneuvers to observe the therapeutic effect of unilateral posterior semicircular canal Benign Paroxysmal Posi tional Vertigo(BPPV)and to explore the role of press-needle in the treatment of unilateral posterior semicircular canal BPPV.The BPPV of semicircular canal after canalith repositioning maneuvers was used as control group.A randomized control led clinical study was conducted with a small sample(70 cases).The main scores were DHI score and duration of residual dizziness,and the recurrence rate in a month.The therapeutic grade of vertigo was used as a secondary evaluation index.The curat ive effect of press needle combined with manual reduction on unilateral posterior semicircular canal BPPV was observed.MethodsA total of 70 patients wi th unilateral posterior semicircular canal BPPV were randomly divided into two groups.The patients were randomly divided into two groups by random numerical table method.The treatment group was treated wi th canalith repositioning maneuversand press acupuncture(3 acupoints once a week for 2 weeks),and the control group was single treated with canalith repositioning maneuvers.After one month of follow-up,if the BPPV symptoms of the posterior semicircular canal were rediagnosed after one week of treatment,then the manual reduction was performed again,and t he observation was suspended if the two reposit ions were still not cured.The data were collected by DHI questionnaire and recorded the general condition of the patients and the duration of residual dizziness.The main evaluation indexes were vertigo disability scale(DHI)and duration of residual dizziness.One-month recurrence rate and vertigo efficacy grade were used as secondary evaluation indexes.The statistics were carried out by software SPSS23.0,and the measurement data were expressed by "(mean+standard deviation").If the comparison between the same time point groups was in accordance with normal distribution and homogeneity of variance,two independent samples t-test were used,otherwise,the non-parametric rank sum test was used.Count data with chi-square test.The difference was statistically significant(P<0.05).The curative effect of acupuncture combined with canalith repositioning maneuvers in treatment ofunilateral posterior semicircular canal BPPV.Resuts(1)Duration of residual dizziness:The duration of residual dizziness in unilateral PC-BPPV was 6.26±3.20 days in the treatment group and 8.47±4.70 days in the control group(P<0.05).The duration of residual dizziness in the treatment group was shorter than that in the control group,and the treatment group could improve the residual dizziness more quickly.(2)DHI score change:In this study,there was no significant difference in DHI scores between the two groups before treatment.After treatment,the DHI scores of both groups were lower than those before treatment,and reached the state of cure within one month of follow-up.One day after treatment,there was significant difference in DHI total score and DHI-E score between the two groups.The treatment group was superior to the control group,and there was no significant difference in DHI-P and DHI-F scores between the two groups.The treatment group could significantly improve the residual dizziness and the quality of life of the patients after 1 day treatment.There was no significant difference in DHI scores between the two groups 1 week after treatment and 1 month after treatment.The curative effect of the two groups was equal.During one month of follow-up,the DHI scores of the two groups all showed a downward trend,and the decrease of the treatment group was faster than that of the control group.(3)Recurrence rate and grade of curative effect of vertigo:In this study,there was no significant difference in recurrence rate between the two groups within one month of follow-up.After the first treatment,the total effective rate was 100%in the treatment group and 94.12%in the control group.The total effective rate in the treatment group was 100%after a weak,and the total effective rate in the control group was 97.06%.There was no significant difference in the grade of curative effect between the two vertigo cases,and the curative effect was equal.(4)Safety and effectiveness:None of the 70 patients observed had an allergic reaction,dizziness,rash,pressure sore,worsening of symptoms,and the like.Of the 70 patients who had been included in the study,only one case was discontinued and the remaining 69 patients were cured within 1 month,with a total effective rate of 98.57%.Research conclusion(1)Canalith repositioning maneuversis effect in treating single-sided pc-BPPV is exact.(2)Combination of press acupuncture and canalith repositioning maneuvers could shorten the duration of residual dizziness and early DHI score.On the basis of canalith repositioning maneuvers,early use of press needle can effectively improve the emotional score of patients with DHI,reduce the severity of residual dizziness,accelerate the relief of symptoms,and thus improve the quality of life of patients.(3)In this study,BPPV of semicircular canal treated with press needle combined with manual reduction could not significantly improve the effective rate and 1 month recurrence rate,but the study showed that press needle could reduce the residual dizziness and shorten the duration of residual dizziness after reduction.It can be used as a supplementary method for the treatment of BPPV in the posterior semicircular canal.(4)Press-needle is safe and effective,less pain,less adverse reaction,easy to be accepted by patients. |
