| Objective1.To explore the correlation between the relevant indexes and the TCM syndromes of diabetic nephropathy,in order to make up for its deficiency by coordinating the modern medical means and assisting the syndrome differentiation mode of the four diagnostic methods of traditional Chinese medicine.2.To explore the significance of Galectin-3 in the diagnosis of diabetic nephropathy, and to search for indicators with high specificity and sensitivity for diabetic nephropathy,so as to improve the accuracy of the diagnosis of diabetic nephropathy.Method50 diabetic nephropathy patients admitted to the first affiliated hospital of guangzhou university of traditional Chinese medicine from April 2018 to June 2019 were selected as the DN observation group, and 20 newly diagnosed type 2 diabetes in the same period were selected as the DM control group, and 20 healthy subjects were selected as the healthy group.According to TCM syndrome differentiation, the observation group was divided into four syndrome types, include Yin deficiency-Dry Heat,Qi-Yin deficiency,Qi deficiency of spleen and kidney,Yin-Yang deficiency.Fasting serum samples from all included patients, frozen 80℃ below zero in the fridge, detectin according to the need to take out at room temperature after melting in advance. In the observation group and control group patients during hospitalization, collect their age, body mass index,DM course, past medical history, personal history, such as general situation, collect the data of routine examination in clinical diagnosis and treatment projects,including:24h urine protein quantitation, urine trace albumin/uric creatinine, fasting glucose, glycosylated hemoglobin, serum creatinine, blood urea nitrogen, total cholesterol, triglycerides, glomerular filtration rate,AST,ALT,TB.The serum Galectin-3 density were determined by enzyme-linked immunosorbent assay(Elisa). The basic information of the included patients and related indexes were tabulated, input into the computer, and SPSS22.0 software was used to establish the database. Then, all indexes were compared between different TCM syndromes in the observation group and the control group to analyze their correlation.ResultsBetween the DN observation group and DM control group, there were no obvious differences in Galectin-3 values,Age,triglyceride,BMI,total cholesterol and AST(P>0.05),but it was significantly difficult in Age,Glycosylated hemoglobin, fasting blood glucose, urea nitrogen, glomerular filtration rate,ACR,24h urine protein quantitation, urinary microalbumin, serum creatinine,ALT and TB(P<0.05).Among the four syndrome types of the DN observation group, there were no obvious differences in Galectin-3 values,Age,triglyceride,BMI,total cholesterol and AST(P>0.05),but it was significantly difficult in Glycosylated hemoglobin, fasting blood glucose, urea nitrogen, glomerular filtration rate,ACR,24h urine protein quantitation, urinary microalbumin, serum creatinine,ALT and TB(P<0.05).There was a correlation between TCM syndrome differentiation and clinical stages of diabetic nephropathy(P<0.05).Conclusion1.There is a correlation between TCM syndrome differentiation and clinical stages of diabetic nephropathy. From the early stage to the late stage of diabetic nephropathy, the TCM syndrome types that account for the main proportion in each stage gradually evolve in the direction of Yin deficiency-Dry Heat,Qi-Yin deficiency,Qi deficiency of spleen and kidney, and Yin-Yang deficiency.2.The clinical routine indexes of urea nitrogen, glomerular filtration rate,ACR,total protein in urine, microalbumin, and serum creatinine are correlated with the TCM syndromes of diabetic nephropathy. The indexes of renal function and urine protein can reflect the evolution of TCM syndromes of diabetic nephropathy, and can be used as the objective reference index of TCM syndrome differentiation.3.GalectinThe significance of Galectin-3 has little significance in the diagnosis of diabetic nephropathy with poor specificity is not clear, and sensitivity.Galectinthe correlation between Galectin-3 has no significant correlation with four and TCM syndrome types syndromes of diabetic nephropathynephropathy is not clear, which requires further research. |
