| Object:To study the correlation between serum cystatin C and lipoprotein a and the early diagnosis and condition evaluation of type 2 diabetes mellitus with acute cerebral infarction patients.Method: From October 2017 to June 2018,fifty patients with type 2 diabetes mellitus complicated with acute cerebral infarction and fifty patients with type 2diabetes mellitus were collected from the first affiliated Hospital of Bengbu Medical College in general medicine,Neurology Department,Emergency internal medicine and endocrinology department.The fifty patients with type 2 diabetes mellitus complicated with acute cerebral infarction are treated as the group of Type 2 diabetes mellitus with acute cerebral infarction(ACI + DM group),and fifty patients with type 2 diabetes mellitus as the group of type 2 diabetes mellitus(DM group),choose the 50 patients of non-type 2 diabetes and the history of acute cerebral infarction in the First Affiliated Hospital of Bengbu Medical College as the control group.The medical history of gender,age,hypertension and heart disease of all the patients was recorded,and 3 ml of venous blood was taken from 3 groups after fasting for 12 h,and use the Automatic biochemical analyzer and matching reagents in the first affiliated Hospital of Bengbu Medical College determine the horizontal of serum total cholesterol(Total Cholesterol,TC)and triglyceride(Triglycerides TG),high density lipoprotein(High Density Lipoprotein HDL),low density lipoprotein(Low Density Lipoprotein LDL)urinary inhibition C(CysC Cystatin C),Lipoprotein a(Lipoprotein a Lp(a)).According to National Institute of Health stroke scale(NIHSS),patients in the ACI+ DM group were evaluated after detailed physical examination and medical history inquiry,and were divided into three categories: mild score 0 ~ 5;Medium grade 5 ~ 15points;Heavy duty 15 to 42.Analysis of the value of serum cystatin C and lipoprotein a in the early diagnosis and evaluation of type 2 diabetes mellitus complicated with acute cerebral infarction.Result: 1.There was no significant difference in gender,age,TC,TG,LDL and HDL between ACI+DM group,DM group and control group,and CysC,Lp(a)levels were not completely the same.The levels of CysC and Lp(a)in the ACI+DM group and DM group were higher than those in the control group,and the levels of CysC and Lp(a)in the ACI+DM group were higher than those in the DM group.2.Logistic regression analysis indicated that cystatin C and lipoprotein a were risk factors for cerebral infarction(P<0.05).3.The combined detection of CysC and Lp(a)significantly increased the positive rate in the diagnosis of type 2 diabetes complicated with cerebral infarction compared with the single detection,and the difference was statistically significant(P < 0.05).4.In the ACI+DM group,the degree of neurological impairment was positively correlated with cystatin C(r = 0.330,P < 0.05)and lipoprotein a(r = 0.298,P < 0.05).Conclusion:1.cystatin C and Lipoprotein a may be associated with the pathogenesis of cerebral infarction and are risk factors for cerebral infarction.2.The combined diagnosis of serum cystatin C and lipoprotein a is of higher value in the early diagnosis of patients with type 2 diabetes mellitus complicated with acute cerebral infarction.3.The levels of cystatin C and lipoprotein a are positively correlated with the levels of cerebral neurological defects,and the levels of serum cystatin C and lipoprotein a are of certain value in the condition evaluation of patients with type 2diabetes mellitus complicated with acute cerebral infarction. |
