Objective:To evaluate the clinical efficacy of fenestration and decompression for the treatment of different types of cystic lesions of the jaw and to explore the factors affecting the healing of cystic lesions.Methods:From January 2014 to March 2019,82 cases of inpatients with oral and maxillofacial surgery in the First Affiliated Hospital of Nanchang University and the Second Affiliated Hospital of Nanchang University,with complete clinical medical records and decompression were collected.The pathological diagnosis of these cases was odontogenic cystic lesions of the jaw,including 36 cases of radicular cysts(RC),18 cases of dentigerous cysts(DC),8 cases of odontogenic cysts(OKC),and unicystic ameloblastoma(UAB)in 20 cases,all underwent decompression,combined with cyst plug for treatment.The patients were followed up on time,and the imaging data of preoperative and postoperative patients were analyzed.The area of four types of cystic lesions before and after decompression was measured by measuring the panoramic radiography(PR).The reduction rate of the radicular cyst group,the dentigerous cyst group,the odontogenic keratocyst group and the unicystic ameloblastoma group was calculated,and the relationship between the patient’s age,initial lesion size,pathological type and reduction speed was analyzed.Results:The cystic area changes of the four cystic lesions after decompression were71.4% ± 14.5% in the RC group,65.0% ± 18.6% in the DC group,70.6% ± 13.3% in the OKC group,and 60.7% ± 16.3% in the UAB group.There was no significant difference between the groups(H = 5.974,P = 0.113).In the RC group(r = – 0.603,P< 0.001),the correlation between the age of the patients and the relative shrinkage rate of the cystic lesion was significant.In the RC group(r = 0.574,P = 0.002),DC group(r = 0.831,P < 0.001),OKC group(r = 0.810,P = 0.015),UAB group(r =0.559,P = 0.010),there was a statistically significant correlation between the initial area of cystic lesions and the absolute reduction of cystic area in the 4 groups.In the UAB group(r = 0.480,P = 0.032),the initial area of the cystic cavity and the area of the cystic cavity were relatively reduced.The correlation was statistically significant.Conclusion:Decompression is an effective method for the treatment of odontogenic cystic lesions.The age of the patient and the initial cyst size may have an effect on the healing rate of the jaw.But there is no significant difference in the decompression effect of different pathological types of cystic lesions of the jaw.
