Aesthetic Reconstruction Of Hairline Defect

Objective:Hairline is the marginal line of hair growth of scalp,and its physiological or pathological defects can bring great physiological pain and psychological trauma to patients.The purpose of this paper is to summarize the surgical methods of applying the expanded scalp flap with natural hairline for cosmetic reconstruction of hairline,so as to provide reference for future clinical work.Methods:In this study,a retrospective analysis was conducted on 23 patients in the plastic surgery ward of the Plastic Surgery Hospital of the Chinese Academy of Medical Sciences from January 2015 to December 2018 who underwent expanded scalp flap cosmetic reconstruction hairline with natural hairline(20 single pedicled flap,3 double pedicled flap),14 cases of males and 9 cases of females,with an average age of 22.1 ± 5.9 years old.The follow-up time was 8-44 months.Result:Of the 23 patients,16 were very satisfied(14 single pedicled flap and 2 double pedicled flap),and 7 were satisfied(6 single pedicled flap and 1 double pedicled flap),patient satisfaction was 100%.Among them,21 patients had good flap survival,and 2 patients(all of whom were single pedicled flap)had venous congestion at the distal end of the flap,including 1 case relieved 3 days after surgery,and the other case had partial skin necrosis at the distal end,healing Phase Ⅱ.The rest of the patients received satisfactory morphological improvement after surgery.Conclusion:Compared with hair transplant and traditional scalp flap transplant without hairline,the expanded scalp flap with hairline has the ability to repair hairline and the skin defects in the outer area of it,the advantages of scar concealment,and the hair distribution,density and hair follicle unit structure of the new hairline in the receiving area are consistent with the natural hairline.Patients have high satisfaction with the surgical effect,which is a good method worthy of promotion.
