Objective:To explore the risk factors of bleeding after tonsillectomy in children by plasma radiofrequency ablation,and to provide theoretical basis for reducing bleeding complications after plasma radiofrequency ablation.Method:A retrospective analysis was made on 841 children undergoing tonsillectomy by radiofrequency ablation from August 2014 to November 2018 in terms of sex,age,average operation time,tonsillar embedding degree,preoperative diagnosis and surgeon’s plasma proficiency and diet management.SPSS20.0 software was used for statistical analysis.Chi-square test was performed on the bleeding rate of different gender,age,embedding degree,preoperative diagnosis and proficiency of surgeons.Mest was performed on the bleeding rate and average operation time of different surgeons.Logistic regression analysis was used for risk estimation and multivariate analysis.Result:There were significant differences in the incidence of bleeding between different preoperative diagnosis and tonsillar embedding degree(The χ2 values were 39.858 and 27.363,respectively,p<0.01).Preoperative diagnosis and tonsillar insertion are high risk factors for postoperative bleeding(OR values were 0.033,p=0.00 and 0.006,p=0.00),Postoperative hemorrhage occurs differently in children of cvdifferent ages,and the incidence of postoperative hemorrhage increases in children over 6 years of age(χ2=10.829,p<0.01).The average operation time of beginners with plasma radiofrequency ablation technology was significantly higher than that of skilled ones(t=34.296,p<0.01).The bleeding rate of skilled patients after plasma surgery was significantly lower than that of beginners.(χ2=6.517,p<0.05)Secondary bleeding accounted for the majority of postoperative bleeding.Conclusion:Preoperative diagnosis and tonsillar embedding degree are independent risk factors for bleeding after tonsillar adenoid plasma surgery;Children over 6-year-old have a higher incidence of bleeding than children under 6-year-old;Beginners have a higher bleeding rate than skilled doctors;Severe tonsillar embedding has a higher bleeding rate than mild and moderate tonsillar embedding.The bleeding rate after tonsillectomy was not related to sex.
