Application Of Ship-shaped Adhesive Tape Fixing In Patients Undergoing Tracheal Intubation

Objective:In order to evaluate the safety and value of the improved ship-shaped fixing on tracheal intubation patients and provide theoretical basis for clinical therapy,we conducted this research to compare the fixed effectiveness,complications,nursing workload and medical expenses about application of ship-shaped adhesive tape fixation on patient undergoing tracheal intubation.Methods:There were 134 tracheal intubation patients who were admitted to the department of surgical intensive care unit of our hospital were included from April first 2018 to August 30th,2018 and from November 5th to December 5th.The patients were randomly divided into experimental group and control group and each group was 67 cases.The experimental group patients received the improved ship-shaped fixing while the control group received traditional X-shaped tape and string fixing.The fixing was performed by skilled nurses who had 2-year working experience or more than 2-year working experience.The oral care of all patients was done three times a day,and the position and depth of the tube was checked and recorded every 4 hours.The tube was fixed originally after oral care.To evaluate the safety and value of the improved 3M tape ship-shaped fixing on tracheal intubation patients according to the comparison of shift rate of the tube,incidence of unplanned extubation,facial skin injury rate,the effect of oral care,incidence of ventilator-associated pneunonia,oral care time and expenses.After establishing the database using EpiData3.1,statistical analysis was conducted by the software of GraphPad Prism 6.0,the eontinuous variables are expressed in terms of x ± s,and tested by t-test and the Mann-Whitney U test,the coumting data was expressed as a percentage,and the comparison was done with a two-tailed chi-square test.The P value<0.05 was considered statistically significant.Results:?There was no statistically significant difference(p>0.05)between age,gender,intubation time,acute physiology and chronic health evaluation system ?(APACHE ?)score and serum albumin level.The tube displacement rate in experimental group was significantly lower than that in the control group(p=0.002).In terms of the occurrence of facial adhesive related skin injury,the experimental group was lower than the control group(p=0.006).There was no statistically difference in incidence of oral odor,redness and mucosal damage between the experimental group and the control group(p>0.05).There was also no statistically difference in incidence of ventilator-associated pneumonia between the experimental group and the control group(p=0.676).The time of oral care,tape replacement and string fixing in experimental groupwas significantly shorter than control group(p<0.0001).The cost of the tape required for the fixation is 0.09 yuan higher in experimental group than control group.The results showed that the improved 3M tape ship-shapedfixing was superior to the traditional X-type fixing,which can secure the endotracheal intubation,reduce nurse workload and complications,save manpower,and not increase the cost.Conclusions:The method of the 3M tape ship-shaped combined with strap fixing method is easy to operate and time-saving.This method can be well fixed,which can significantly reduce the incidence of skin damage.The incidence of ventilator-associated pneumonia is not increased with this method.It also can improve patient comfort,reduce nursing workload,improve the quality and efficiency of nursing and reduce the medical expenses.
