Effect Of Continuous Nursing Model Based On IKAP Theory On Patients With Peptic Ulcer Bleeding

ObjectiveTo construct a systematic and complete nursing model for patients with peptic ulcer bleeding by using IKAP theory.To explore the intervention effect of continuous nursing model based on IKAP theory on patients with peptic ulcer bleeding.Methods47 patients with peptic ulcer bleeding treated in Department of Gastroenterology,a third Class A Hospital in Changsha City from 2018.06 to2018.08 were selected as the control group,wihle 47 patients from 2018.10 to2018.12 were selected as intervention group.Both groups were given standardized,reasonable and appropriate medication and treatment during hospitalization and after discharge.Routine nursing was carried out in the control group,and IKAP continuous nursing intervention was carried out in the intervention group on the basis of the control group.General data survey,clinical data survey,health promotion lifestyle scale(HPLP-II)and Hamilton anxiety and depression scale(HAMA,HAMD)were completed in 3 days after admission.Besides,all patients were evaluated again with the HealthPromotion lifestyle scale(HPLP-II)and Hamilton anxiety and Depression scale(HAMA,HAMD)3 months after discharge,and rescrutinized hematuria,Nitrogen,hemoglobin,ulcer healing and the occurrence of recurrent bleeding of those patients collected..The statistical analysis of the obtained data was carried out by SPSS20.0 statistical software,and the difference was statistically significant if P < 0.05.Results1.There were no significant differences in general data,health promotion lifestyle,anxiety and depression,hemoglobin,urea nitrogen and fecal occult blood test between the intervention group and the control group before intervention(P > 0.05).2.There were significant differences in blood urea nitrogen,hemoglobin and ulcer healing between the intervention group and the control group(P <0.05).The rebleeding rate in the intervention group(2.1%)was lower than that in the control group(12.8%).The difference was statistically significant(P <0.05).3.The score of health promotion lifestyle in the intervention group was16.66 ±0.50,which was significantly higher than that in the control group(14.92 ±0.46),and the dimensions of the intervention group were higher than those in the control group.There was significant difference between the two groups(P < 0.05).4.The scores of depression and anxiety in the intervention group were(7.43 ±2.58),(5.13 ±1.77),which were significantly lower than those in the control group [(8.77 ±2.66),(6.62 ±2.95)].The anxiety and depression assessment of the patients was also lower than that of the control group,and there was significant difference between the two groups(P < 0.05).Conclusion1.The continuous nursing model of peptic ulcer bleeding patients was successfully constructed based on IKAP theory and Pender health promotion model.2.The continuous nursing mode based on IKAP theory can make the patients with peptic ulcer bleeding get purposeful,planned and systematic continuous nursing during hospitalization and after discharge.3.The continuous nursing mode based on IKAP theory can improve the health and life style,reduce anxiety and depression and reduce the rate of rebleeding of patients with peptic ulcer bleeding...
