| ObjectiveThrough the treatment of patients with chronic atrophic gastritis(chronic atrophic gastritis,CAG)with spleen deficiency liver depression and blood stasis,the clinical symptoms,gastroscopy pathological changes and OLGA staging(operative link for gastritis assessment)and OLGIM(operative link for gastric intestinal metaplasia assessment)associated with the risk of gastric cancer can be used to objectively evaluate the clinical efficacy of Jianpi Shugan Huoxue Jiedu method in the treatment of CAG,especially the effectiveness and safety of pathological effects.MethodsA total of 64 patients of CAG which met the diagnostic criteria were selected in Guangdong Provincial Second Hospital of Traditional Chinese Medicine during March 2017-July 2018,were randomly divided into treatment group and control group with 32 in each group.Jianpi Shugan Huoxue Jiedu detoxification was used in the Treatment group,whereas Weifuchun Table was given to the control group.And 2 cases fell off in the treatment group and 1 cases fell off in the control group.The course of treatment in both groups was 6 months.After the treatment,the clinical symptoms,gastroscope pathology,OLGA and OLGIM staging of the two groups were observed before and after treatment to evaluate their curative effect.All data are analyzed by SPSS 19.0 statistical software.Results1.Pathological effect:① Gastric mucosal atrophy:There were significant differences in the pathological degree of atrophy of gastric sinus,gastric angle and gastric body before and after treatment in the two groups(P<0.05).The treatment group can reverse the atrophy of gastric sinus,gastric horn and stomach body,the total effective rate was 93.33%,89.29%and 85.71%respectively,the control group was 70.97%,68.97%and 75%,and the curative effect of the treatment group was better than that of the control group(P<0.05).②Intestinal metaplasia:There were significant differences in the degree of intestinal pathology improvement between the two groups before and after treatment in gastric sinus,gastric angle and gastric body(P<0.05).The treatment group could reverse the degree of intestinal epithelial metaplasia of gastric sinus,gastric angle and gastric body,the total effective rate was 78.26%,85%and 73.33%respectively,the control group was 66.67%,71.43%and 71.43%,and there was no significant difference between the curative effect and the control group(P>0.05).③ Gastric Dysplasia:There were significant differences in the pathological improvement of dysplasia between the two groups before and after treatment(P<0.05),The results indicated that the treatment group and the control group had the same curative effect in improving the dysplasia of gastric mucosa.Because Jianpi Shugan Huoxue Jiedu method has a certain prospect in reversing gastric mucosal dysplasia,and the number of cases of heterosexual hyperplasia is small,it still needs further study.④Chronic inflammat:There was a significant difference in the pathological degree of chronic inflammation between the two groups before and after treatment in 3 parts of gastric sinus,gastric body and gastric angle(P<0.05),the curative effect of the treatment group was better than that of the control group(P>0.05).⑤Activitiy:In gastric sinus and gastric body,there were significant differences in the pathological degree of chronic activity between the two groups before and after treatment(P(0.05).The method can obviously improve the inflammatory activity of mucosa by Jianpi Shugan Huoxue Jiedu method.⑥The degree of improvement of body pathology:The total pathological points and differences of the treatment group were 7.57±7.10%21.93±5.89,the control group was 13.90±7.15、14.81±6.53,and the overall pathological effect of the treatment group was significantly different from that of the control group(P<0.05).In general,Jianpi Shugan Huoxue Jiedu method is superior to gastric rehydration in reversing the histopathological of gastric mucosa.2.Clinical symptoms:After 6 months of treatment,both groups were better than before treatment in terms of clinical syndromes(P<0.05),and the treatment group was more effective than the control group with significant differences(P<0.05).The comprehensive effect rate of clinical syndromes in the treatment group(93.33%)was significantly higher than that of the control group(70.97%)(P<0.05),and it is suggested that Jianpi Shugan Huoxue Jiedu Method was more effective than weifuchun in the curative effect of clinical syndrome.3.The risk of cancer:After the treatment,the stratification of OLGA and OLGIM of Both groups was decreased(P<0.05),and the decline of treatment group was more pronounced than that of the control group(P<0.05).Thus,it is explained that the treatment group has better efficacy in reducing the risk level of OLGA and OLGIM.ConclusionThrough statistical analysis,the treatment group was superior to the control group in improving clinical symptoms,pathological status and cancer risk.It is shown that Jianpi Shugan Huoxue Jiedu method on the treatment of chronic atrophic gastritis not only can improve symptoms,but also decreased the level of atrophy and IM.In the meanwhile,it significantly reduced the stratification of OLGA and OLGIM to reduce the risk of cancer,reflecting the effectiveness and generalization of the treatment of CAG by Jianpi and Shugan Huoxue detoxification.Consequently,this method embodies the effectiveness and worth promoted widely. |
