ObjectiveOptic atrophy is the final outcome of a variety of optic nerve diseases.The causes are various,and the result is severely impaired or even completely lost.At present,there is no effective treatment.Therefore,We plan to present an objective evaluation of the protection effect of Shuxuetong injection of which main ingredients are Dilong and Shuizhi extraction on the remaining visual function of optic atrophy through retrospective clinical observation,to confirm its effectiveness and safety,to provide a new treatment for optic atrophy,and to expand the clinical indications of Shuxuetong Injection.MethodsA retrospective analysis was carried out on the clinical data,the patient of the diagnosis of optic atrophy and coform to the standard of including and exclusion criteria were choiced between January 2014 and December 2018 in Guangdong Provincial Hospital.Patients were divided into two groups,Shuxuetong injection treatment group(observation group)and Traditional Chinese medicine treatment without Dilong and Shuizhi and its extraction(control group),The changes of the best corrected visual acuity,visual field,visual evoked potential,retinal nerve fiber layer thickness and blood stasis syndrome in the two groups were retrospected in one month,two months and three months after treatment,and the changes of blood stasis syndrome after 1 month of treatment.ResultsThis study included 91 cases,including observation group of 47 cases,control group of 44 cases,comparing the comparison in patients of two groups such as gender,age,disease course and blood stasis syndrome,There were no difference by statistical analysis(P>0.05).1.Best Corrected Visual Acuity(BCVA):The BCVA of observation group at different time before treatment,1 month after treatment,2 months after treatment,and 3 months after treatment were 0.32±0.14、0.43±0.16、0.42±0.18、0.34±0.16,the BCVA of control group were 0.30±0.13、0.34±0.15、0.33±0.14、0.31±0.13.The BCVA of observation group in 1 month and 2 months after treatment were higher than that before treatment(P<0.05).The control group was improved after 1 month treatment compared with before treatment(P<0.05).The BCVA of the observation group in 1 month and 2 months after treatment were better than that of the control group(P<0.05).2.Perimetry:In the observation group,the visual field defects(unit:dB)at different time before treatment,1 month after treatment,2 months after treatment,and 3 months after treatment were-19.29±3.15、-16.40±3.49、-17.28±3.32、-18.28±2.37,the visual field defect degree of control group were-19.78±3.51、-18.46±3.81、-19.04±3.17、-19.79±3.42.The visual field defect of observation group in 1 month and 2 months after treatment were lower than that before treatment(P<0.05).Compared with before treatment,the control group showed a significant decrease in 1 month after treatment(P<0.05).The degree of visual field defect in observation group were lower than that in control group at each observation time point after treatment(P<0.05).3.visual evoked potential examination:The P100 latency(unit:ms)of the observation group before treatment,1 month after treatment,2 months after treatment,and 3 months after treatment were 136.56±8.38、137.90±11.68、128.90±11.68、132.53± 11.48,The P100 latency of control group were 135.98±8.08、134.81±6.22、132.01 ±7.02、134.11±6.57.Compared with before treatment,the latency of P100 in the observation group were shortened in 2 months after treatment(P<0.05).There were no significant difference at each observation time point after treatment compared with that before treatment in the control group(P>0.05).There were no significant difference in the latency of P100 between the two groups after treatment(P>0.05).The amplitude of P100(unit:μV)of the observation group before treatment,1 month after treatment,2 months after treatment,and 3 months after treatment were 4.69±1.23、4.98±2.37、6.36±1.48、5.10± 1.55,The amplitude of P100 of control group were 4.53±0.90、4.22±1.92、5.03±1.54、4.89±1.46.Compared with before treatment,The amplitude of P100 in the observation group were shortened in 2 months after treatment(P<0.05).There were no significant difference at each observation time after treatment compared with that before treatment in the control group(P>0.05).The amplitude of P100 of the observation group in 2 months after treatment were significantly better than that of the control group(P<0.01).4.OCT:The retinal nerve fiber layer thickness(unit:μm)at the different time points of the observation group before treatment,1 month after treatment,2 months after treatment,and 3 months after treatment were 78.04± 13.64、77.38± 13.43、77.26± 14.14、77.62± 16.22,The thickness of retinal nerve fiber layer in the control group were 77.23±9.29、77.66±8.67、77.32±8.52、77.27±11.84.There were no significant difference between each group after treatment(P>0.05).There were no significant difference in the thickness of retinal nerve fiber layer between the two groups after treatment(P>0.05).5.Symptoms of blood stasis:The scores of blood stasis syndrome in the observation group before and after treatment were 8.23±4.00 and 4.93±2.99.The scores of blood stasis syndrome in the control group were 7.41±2.78 and 5.59±2.90.The blood stasis syndrome in the two group improved after treatment compared with that before treatment(P<0.05).There was no significant difference in the blood stasis syndrome score between the two groups after treatment(P>0.05).6.Observation of safety indicators:During the treatment,allergic reactions,side effects,adverse reactions and transfusion reactions occurred in both groups were all zero.Conclusion:1.Shuxuetong injection of which main ingredients are Dilong and Shuizhi extraction has a good intervention effect on the best corrected visual acuity,visual field and visual evoked potential of patients with optie atrophy.It can improve patient’s vision,reduce the visual field defect,shorten the P100 latency and increase the amplitude.And Shuxuetong injection has the function of breaking through the collaterals and improving the blood stasis syndrome of patients.2.For morphological examination,it has not been found that Shuxuetong injection combined with traditional Chinese medicine treatment or conventional Chinese medicine treatment can increase the thickness of retinal nerve fiber layer.3.There were no adverse reactions and serious adverse events in the two groups during and after the treatment,which indicated that the two treatment methods and drug use were safe. |
