ObjectiveTo explore the effects of exercise therapy combined with infrared therapy on the improvement of joint mobility in patients with rheumatoid arthritis(RA),at the same time,to compare the therapeutic effects of conventional drug therapy and it combined with exercise therapy.So as to provide new treatment ideas for the clinic work.MethodsFrom August 2017 to August 2018,117 patients,who were diagnosed RA,in the Department of Rheumatoid Rehabilitation of the First Hospital of Jiaxing were recruited.The patients were randomly divided into exercise therapy group,exercise therapy combined with infrared therapy group and conventional drug treatment group,39 cases each.Three groups were all accepted conventional drug treatment,but the exercise therapy group added exercise therapy.On the basis,the exercise therapy combined with the infrared treatment group was treated with far-infrared therapy.The duration of morning stiffness,disease activity score,grip strength,hand function,and active range of motion(AROM)of each subject were recorded before treatment,after 12 weeks and 24 weeks,respectively.Differences between treatments.Statistical analysis was performed using one-way analysis of variance,rank sum test,and chi-square test.Results1.Patients characteristicsA total of 98 patients completed the follow-up study,including 32 in the exercise therapy group,33 in the infrared combined exercise therapy group and the conventional drug treatment group.There were no significant differences in gender,age,duration of disease,hormonal use,and biologic use between the three groups.2.Morning stiffness timeAfter 12 weeks of treatment,the morning stiffness time of exercise therapy combined with infrared treatment group decreased significantly,and the difference was statistically significant compared with the exercise treatment group and the conventional drug treatment group(P<0.05).After 24 weeks of treatment,the improvement of morning stiffness duration in the exercise group and exercise therapy combined with the infrared treatment group was better than that in the drug treatment group,the difference was statistically significant(P<0.05).3.The disease activity scoreAfter 12 weeks of treatment,the Patient Global Assessment of exercise therapy combined with infrared therapy was lower than that ofexercise therapy and drug therapy,and the difference was statistically significant(P<0.05).But there was no significant difference between the exercise group and the drug treatment group(P>0.05).After 24 weeks of treatment,the PGA scores of the infrared combined exercise therapy group and the exercise treatment group were significantly lower than the drug treatment group(P=0.00).However,there was no difference in the number of swollen joints,number of tender joints,Erythrocyte Sedimentation Rate,and DAS28 score(Disease Activity Score 28)after treatment for 12 weeks and 24 weeks(P>0.05).4.Range Of MotionAfter 12 weeks of treatment,the exercise therapy combined with infrared therapy and the exercise therapy in both wrist and dorsiflexion of the ankle joint was better than the drug treatment group(P<0.05).After24 weeks of treatment,exercise group and exercise therapy combined with infrared treatment group,the range of motion that double wrist joint extension,palm flexion,elbow joint mobility,shoulder flexion,extension,abduction,and knee joint was better than the drug treatment group(P<0.05),but there was no significant difference between the two groups(P>0.05).5.Hand functionAfter 24 weeks of treatment,the grip strength in exercise therapy group,exercise therapy combined with infrared treatment group cansignificantly improve of RA patients,and significantly better than the drug treatment group(P <0.05).At the same time,the Signals Of Functional Impairment(SOFI)in the treatment After 12 weeks,that was a significant difference from the drug treatment group(P<0.05).Conclusions1.Exercise therapy combined with infrared therapy can significantly improve the morning stiffness time and PGA score of patients,that is superior to exercise therapy group and drug treatment group.2,Exercise therapy and infrared combined exercise therapy have no significant effect on the disease activity of RA patients,but it will not aggravate the disease activity.3,Exercise therapy combined with infrared therapy and exercise therapy can assist conventional drug treatment to improve the patient’s condition,while long-term adherence to exercise therapy can significantly improve the range of motion involved in RA patients joints and grip strength,hand function.It should be worthy of further clinical promotion.
