| BackgroundIn recent years,the delay in childbearing age has become a global problem,the number of pregnant women is increasing year by year,and the pregnancy outcome of advanced maternal age(maternal age ≥ 35 years of age at delivery)is also increasingly affected by clinical obstetricians with the increasing number and proportion of advanced maternal age.The risk of perinatal births adverse outcomes in advanced maternal age such as premature birth,macrosomia,stillbirth is significantly increased.Therefore,understanding the high risk factors of pregnant and puer-peral period can provide a scientific basis for the protection of prenatal and postnatal care,and guidance antenatal care of advanced maternal age,improve the quality of obstetric medical technology and improve the perinatal infant outcome of advanced maternal age.ObjectiveTo analyze the pregnancy outcome in advanced maternal age and explore the related factors of adverse pregnancy outcome of the perinatal of advanced maternal age,aiming to guide medical institutions at various levels and maternal and child health care centers to perfect maternal maternal management and reduce the incidence of pregnancy complications,guarantee a good birth and good care and improve the perinatal births outcomes in advanced maternal age.MethodsThe clinical data of 456 case of advanced maternal age who gave birth in anyang maternal and child health care hospital from January 2016 to December 2018 were selected and retrospective analysis,they were used as the elder age group(age 35~46 years old),and selected 180 case of non-advanced maternal age who gave birth in our hospital as the non-elder age group(age 22~34 years old).The general clinical data,pregnancy complications,mode of delivery,childbirth and puerperal complications,mental state and perinatal births outcomes of the 2 groups were compared.The advanced maternal age were divided into adverse outcome group(n=72)and good outcome group(n=384)according to whether the perinatal child has a bad pregnancy outcome.The univariate and logistic regression analysis of dichotomous variables were used to screen for risk factors for adverse pregnancy outcomes of the perinatal of advanced maternal age.Results1.The incidence of pregnancy with anemia in the elder age group was significantly higher than the non-elder age group(P<0.01).2.The incidence of hypertensive disorders complicating pregnancy,premature rupture of membrane and abnormal amniotic fluid in the elder age group was significantly higher than the non-elder age group(P<0.05 or P<0.01).3.The cesarean section rate of the elder age group was significantly higher than that of the non-elder age group(P<0.01).4.The scores of self-rating anxiety scale(SAS)and self-rating depression scale(SDS)in the late pregnancy of the elder age group were significantly higher than those in the early pregnancy and the non-elder age group(P<0.01).5.The incidence of fetal intrauterine distress,preterm neontes,fetal malformation and asphyxia of newborn in the elder age group was significantly higher than the non-elder age group(P<0.05).6.Logistic regression analysis showed that the age,gestational diabetes mellitus,hypertensive disorders complicating pregnancy and placental abruption were positively correlated with the occurrence of adverse pregnancy outcome of the perinatal of advanced maternal age(P<0.05),which were the risk factors for adverse pregnancy outcome of the perinatal of advanced maternal age(OR>1).Conclusion1.The incidence of adverse pregnancy complications,cesarean section rate,incidence of adverse psychological status during pregnancy and adverse pregnancy outcomes are significantly higher in advanced maternal age than in non-advanced maternal age.2.The age,gestational diabetes mellitus,hypertensive disorders complicating pregnancy and placental abruption are risk factors for adverse pregnancy outcomes of the perinatal of advanced maternal age. |
