BackgroundSince 1989,anesthesiology has been recognized as a second-level medical discipline,and the department of anesthesiology has become a first-level clinical department.The department of anesthesiology has developed rapidly in China,playing a decisive role in clinical anesthesia,emergency and resuscitation,critical illness surveillance,and pain clinical.Anesthesiology has become an important part of clinical medicine,and the department of anesthesiology has developed into an important clinical specialty reflecting the comprehensive ability of medical institutions.With the development of anesthesiology and the increasing of anesthesiology scope,hospitals in first-tier cities such as Beijing,Shanghai have set up posts of nurse anesthetists and carried out anesthesia nursing work to relieve the burden of anesthesiologists.nurse anesthetists are nurses specializing in anesthesiology nursing work.Henan province is located in the central plains,the development of anesthesiology nursing work is unbalanced.The cultivation and use of nurse anesthetists need to be unified and standardized.ObjectiveTo understand the staffing,job responsibilities,clinical working ability and the current situation of training of nurse anesthetists in tertiary hospitals in Henan province,and to analyze the role position,the scope of work and ability requirements of nurse anesthetists,in order to provide a theoretical basis for the standardized construction of the anesthesia nursing team.MethodsUsing the method of cross-sectional survey,through the literature review,according to relevant national policy requirements,consulting experts in the field of anesthesia,used "The work scope of nurse anesthetists" questionnaire and designed "The clinical working ability of nurse anesthetists" questionnaire,to investigate the job responsibilities and clinical working ability level of nurse anesthetists in tertiary hospitals in henan province.Questionnaire on the work scope of nurse anesthetists includes three parts: basic information of the hospital,human resources of nurse anesthetists,and post setting of nurse anesthetists.The survey was conducted on the heads of anesthesiology departments in 62 tertiary hospitals in Henan province.The Cronbach coefficient of the questionnaire in preliminary investigation is 0.928.Questionnaire on clinical working ability of nurse anesthetists: Delphi method was used to determine the content of the questionnaire.including professional theoretical knowledge,specialized nursing skills,airway management and first aid ability,sense of responsibility and so on,a total of 30 items.The survey was conducted on 205 nurse anesthetists from 62 tertiary hospitals.The Cronbach coefficient of the questionnaire in preliminary investigation is 0.948.Use the IBM SPSS Statistics 19.0 to process the data,use the mean,standard deviation and composition ratio for statistical description,and use the one-way anova to analyze the factors influencing the clinical working ability of nurse anesthetists.The difference was statistically significant at P<0.05.Results1.General hospital conditions: A total of 62 tertiary hospitals were investigated in Henan province,including 47 general hospitals(75.81%),7 specialized subject hospitals(11.29%)and 8 traditional Chinese medicine hospitals(12.90%).2.Setting up of nursing unit in anesthesiology department: 55 hospitals(88.71%)have deployed nurse anesthetists,49 hospitals(79.03%)have nurse anesthetists working in the anesthesia induction and recovery rooms,22 hospitals(35.48%)were staffed with general nurses,25 hospitals(40.32%)have nurse anesthetists working in the operating rooms,and 15 hospitals(24.19%)were staffed with outpatient nurses of anesthesiology department.There were a total of 1099 operating tables and 93 nurse anesthetists working in the operating room.The ratio of nurse anesthetists working in the operating room to operating tables was 0.08:1.3.The present situation of nurses human resources allocation in anesthesiology department: The average age of nurse anesthetists are 34.37±8.52 years.The average professional working years are 7.21±7.13 years.Bachelor degree(57.56%),The professional title of senior nurse(42.93%).4.Anesthesiology department nursing work: Among the 62 hospitals,55 hospitals(88.71%)have carried out anesthesia nursing work.The management rate of narcotic drugs was 90.32%,the management of instruments and equipment(87.10%),the management of department infection was 83.87%,the position of anesthesia and resuscitation patients was 80.65%,the observation and recording of monitoring indicators was 80.65%.The head of anesthesiology department had the greatest demand for the post of nurse anesthetists working in the operating room,which was 94.52%,while the actual working rate of nurse anesthetists working in the operating room was 71.94%.5.The clinical working ability score of nurse anesthetists was 83.29±19.39.Among the general demographic characteristics of nurse anesthetists,age,working years,educational background and professional title were significantly correlated with clinical working ability score(P < 0.05).6.The training of nurse anesthetists was in-hospital training accounting for 72.20%.Most nurses rotated to the operating room(68.78%)for professional learning.ConclusionsThe allocation of human resource of nurse anesthetists in tertiary hospitals in Henan Province is seriously insufficient,with a single type of personnel allocation.Nurse anesthetists are mainly engaged in basic work and lack of specialized education.Therefore,the standardized training and qualification certification system for nurse anesthetists needs to be unified and improved.
