Objective: To evaluate the efficacy of all-arthroscopic matrix_induced autologous cartilage transplantation under all arthroscopy in the treatment of cartilage defects.Methods:A total of 22 patients(22 knees,14 males and 8 females)were studied from August 2016 to August 2018;The average age was 30,the average BMI index was25.3 ± 4.25kg/m 2),was treated with all-arthroscopic matrix_induced autologous cartilage transplantation under all arthroscopy in the first affiliated Hospital of Shenzhen University.The lysholm score,VAS score,Z value and T2 value depending on T2 Mapping MRI were calculated preoperation,postoperative(PO)3,6 and 12 months after operation.With the single factor and multi-level difference analysis,oneway ANOVA was used to test the difference.Examine the relationship between each two months The corresponding F value and significance were obtained to demonstrate the effectiveness and trend of tissue engineered cartilage transplantation under all arthroscopy in the treatment of cartilage defects.Results:All patients were followed up effectively and the results are as follows.One patient suffered from mild joint swelling and pain,accompanied by limited flexion and extension,and returned to normal after being treated by a convalescent.The graft adheres well 3 months after operation,edema signal disappeared at 6 months after operation,and the signal of cartilage graft remained stable in the following 1 year.One year after operation,the value of T2 Mapping Z value was significantly higher than that before operation(p <0.05),and at 6 months after operation,it was significantly higher than that at 3 months after operation(0.72±0.05 vs 0.42±0.06,p<0.05).The lysholm score increased significantly 3 months after operation(80.00±3.87 vs 59.18±4.92,p<0.05).The lysholm scores at 12 months after operation was significantly higher than that at 3 months(90.64±1.84 vs 80.00±3.87,p<0.05).The VAS scores of the patients decreased significantly 1 year after operation(0.32 ± 0.48 vs5.36 ±0.95,p<0.05).Conclusion:The method has good curative effect on the treatment of the cartilage defect of the knee joint under the all arthroscope,whether the functional score or the image score is obviously improved.It can quickly improve the symptoms of discomfort before operation and accelerate the recovery process of patients at the same time.The change of ligament signal in MRI is an important index of individualized rehabilitation program,and a reasonable rehabilitation plan is an important step in treatment.Compared with the traditional open second generation ACI,it can obviously reduce the complications of incision,including incision infection,scar hyperplasia,hemorrhage reduction,and decrease the coordination of knee joint and nerve atrophy caused by the injury of small nerve.It is worth popularizing.
