Objective The aim is to evaluate the health-related quality of life of patients with chronic diseases in Gansu,Hebei,Sichuan and Zhejiang,and to find out the influencing factors of the health-related quality of life of patients with chronic diseases in Gansu,Hebei,Sichuan and Zhejiang.On the basis of analysis,scientific,suitable and feasible suggestions will be advised in the end.Methods From May 2017 to May 2018,in Gansu,Hebei,Sichuan,Zhejiang province,a combination of stratified sampling and random sampling was used to extract patients with chronic diseases from one tertiary hospital and two county hospitals in each province as research targets.Questionnaires were used to collect information about the basic conditions,employment and economic status,and health protection status of patients with chronic diseases in each province.The European Five-Dimensional Health Scale(EQ-5D-5L)was used to measure the health-related quality of life of patients with chronic diseases.The health-related quality of life of patients with chronic diseases was compared by chi-square test and non-parametric test.The logistic model and Clad model were used to evaluate the health-related quality of life of patients with chronic diseases in Gansu,Hebei,Sichuan and Zhejiang province from the structural dimension and overall dimension,and the influencing factors were identified.Results The health-related quality of life of patients with chronic diseases in Zhejiang Province was significantly better than that of the other three provinces.This may be related to the reasons of Zhejiang's beautiful natural environment,rapid economic development,high economic level and high humanistic quality.Health-related quality of life of patients with chronic diseases in Gansu,Hebei,Sichuan and Zhejiang province differs in usual activities dimension,pain or discomfort dimension,anxiety or depression dimension.(P<0.05)There are more people with problems of pain or discomfort and anxiety or depression.This is due to the long process,difficult to cure,easy recurrence,heavy economic burden,or physical discomfort caused by diseases or drugs,or mental unhappiness.Factors that negatively affect health-related quality of life in patients with chronic diseases in four provinces include age,BMI sort(emaciation),occupation(institutions),employment status(unemployment),income level(below 500,500-2000),and economic sources(government)bailout).(P<0.05)Factors that have a positive impact on health-related quality of life in patients with chronic diseases in four provinces are gender(male),BMI sort(normal,overweight),marital status(married),education(junior high school and below),occupation(farmers,retired),economic sources(salary,retirement,family supply),medical insurance(urban workers,urban residents,new rural cooperatives),health examinations(yes).(P<0.05)Conclusion It is recommended to pay attention to the elderly,women,and unemployment patients with chronic diseases and to improve their health-related quality of life.It is recommended to strengthen health education and health promotion and to improve the mental health of patients with chronic diseases.It is recommended to pay attention to poor low-income patients with chronic diseases and to improve the implementation of health poverty alleviation in a more precise way.It is recommended to improve the quality of health examination services and to strengthen the awareness of health management of patients with chronic diseases.It is recommended to continue to narrow the regional health differences and to promote the construction of healthy and strong countries.
