Part1 The design and implementation of a new portable digestive endoscopic systemObjective : Currently,most of clinical common digestive endoscopes are foreign products,such as Japan’s Olympus,Fujifilm and Pentax Corporation and so on.These conventional endoscopy systems are bulky,are difficult to assembly and transport and have high requirements for electricity and water,which greatly limit the application of the conventional endoscopic system in the special environment,such as emergency,intensive care unit and grassroots medical institutions.Therefore,it is of great significance to develop a new portable digestive endoscopic system that has the same function as conventional digestive endoscopy and is suitable for use in various special circumstances,so as to improve the diagnostic and therapeutic ability of digestive system diseases(such as digestive tract perforation,bleeding,foreign bodies,etc.)under special circumstances.Method: According to the environmental and diagnostic requirements,and combining with the existing endoscopic technique,our hospital has designed and developed a new type of portable digestive endoscopic system.The new portable digestive endoscopic system should have the same function as ordinary endoscopy without the need of washing equipment.It can be vehicle-mounted,carried and towing.It can be rapidly expanded and assembled without the complicated steps of conventional soft endoscopy cleaning and disinfection.Results: After repeated tests,we have successfully developed a new portable digestive endoscopic system(type APG-1).The total weight of the portable digestive endoscopic system is less than 30 Kg,the external size is 66×40×30cm(+-5cm),the positive pressure of the water and air pump is 60 ~ 95 Kpa,the air flow is more than3L/min,the negative pressure of the air pump is-30 ~-60 kpa,and the rated power of the equipment is 220 VA.The new portable endoscopic system mainly includes box-type storage and transportation system,endoscopic system,case management and endoscopic control system,etc.The system is light in size,is convenient in expansion and storage,has good operating performance and safety,and is suitable for emergency,intensive care unit,ambulance,grass-root medical organization and other special environments.The box-type storage and transportation system has a large capacity,which can accommodate the entire portable endoscopic system without additional attachment.It is convenient to carry,isstrong and light,can effectively protect all systems in the box,and suitable for transportation.The endoscopic system includes the electronic upper gastrointestinal endoscopy,the host computer(image processor and cold light source,type APM-1)and related accessories,which is equivalent to the clinical endoscopic diagnosis and therapeutic function of the clinical endoscopy.The case management and endoscopic control system is equipped with endoscopic workstation software and endoscopic control software,which can store endoscopic images and videos,establish cases and produce case reports.Conclusion:The new portable digestive endoscopic system can enter the digestive tract through the natural orifice of human body for diagnosis and treatment,which is suitable for emergency,intensive care unit and grass-root medical organization.The new portable endoscopic system is convenient in expansion and storage,and is without the complicated steps of cleaning and disinfection.Part2 The animal experiment of the new portable digestive endoscopy system on the upper gastrointestinal endoscopyObjective : We will evaluate the operation performance and safety of the new portable digestive endoscopy system in upper gastrointestinal endoscopy by animal experiments.Method: Animal experimental study uses the method of parallel comparison,non-inferiority.We select two healthy Barna pigs as the research object.Ten endoscopic physicians perform upper gastrointestinal endoscopy with Olympus endoscopy(type GIF-Q260)and portable endoscopy.We will record the time of expansion and storage of portable endoscope system,the equipment time of endoscopic one-time use of the sheath system and the time of absorbing 400 ml saline,and evaluate the instrument quality(biopsy channel,water-air suction,rigid and flexible endoscopic body,knob operation,bending,field of view,machine tightness,etc.),image quality(image clarity,image distortion,image color resolution,image illumination,etc.)and safety by quantitative scoring index.Results: Animal experiment shows that the average time of expansion and storage of the new portable endoscopic system are respectively(110.24±8.93)seconds and(91.33±11.59)seconds,and the equipment time of endoscopic one-time use of the sheath system is(233.48±17.06)seconds.The time of absorbing 400 ml saline of the new portable endoscope and Olympus endoscopy are(56.44±5.18)seconds,(33.71±3.56)seconds(P <0.001).The two kind of endoscopy’s ability of absorbing water and air has statistically difference(P < 0.05),while the operating performance,such as the biopsy channel,lens barrel nut is soft,knob,endoscopy bending,visual field,the whole machine sealing,image resolution,image distortion,color resolution and image illuminance,and the occurrence of adverse reactions have no statistical difference(P > 0.05).Conclusion:In addition to the water-air suction function,there is no statistical difference between the new portable endoscopic system and the commonly used Olympus endoscopic system in terms of other instrument quality,image quality and safety.Therefore,the new portable endoscopic system is safe and effective for upper gastrointestinal endoscopy.
