| Objective: To observe the clinical effect of Bushen Huoxue Prescription combined with prednisone and aspirin in the treatment of recurrent spontaneous abortion of kidney deficiency and blood stasis type,and to explore the related mechanism of aspirin and prednisone combined with Bushen Huoxue Prescription,so as to provide a method for clinical treatment of recurrent spontaneous abortion.Methods: From October 2016 to December 2017,ninety patients with recurrent spontaneous abortion(RAS)of kidney deficiency and blood stasis type were selected,in the outpatient and inpatient department of gynecology and obstetrics department of Hubei Provincial Hospital Of TCM.Ninety patients were divided into treatment group(30 cases),control group 1(30 cases)and control group 2(30 cases)according to the principle of informed consent.The treatment group and control group 1 were given aspirin 12.5mg,prednisone acetate 2.5mg,po,qd,for three months.Observing the pregnancy of patients,the treatment group was given Bushen Huoxue Chinese medicine combined with luteal support treatment afterintrauterine pregnancy;the control group 1 was given luteal support treatment;the control group 2 was given Bushen Huoxue treatment after pregnancy.The dosage and frequency of Chinese herbal medicine/luteal support therapy were adjusted according to the relevant test results and clinical symptoms of patients,and gradually decreased to discontinuation after 12 weeks of gestation.During the treatment,the improvement of TCM symptoms,pregnancy success rate,Human Chorionic Gonadotropin(beta-HCG)and Progesterone(P)levels in early pregnancy were observed.Results: 1.Statistical analysis showed that age,the frequency of spontaneous abortion and average days of spontaneous abortion in the three groups with no significant differences(p>0.05).2.Pregnancy Outcome of three groups: Comparing the success rate of fetal protection between the treatment group and the control group,25 cases in the treatment group were successful,3 cases in failure,the success rate was89.3%;16 cases in the control group 1 were successful,14 cases in failure,the success rate was 53.3%;19 cases in the control group 2 were successful,11 cases in failure,the success rate was 63.3%.The total effective rate of the treatment group was significantly higher than that of the control group(p < 0.05).3.Comparing the effectiveness of TCM syndromes between the treatment group and the control groups: 20 cases were cured,6 cases were effective,2 cases were ineffective,the effective rate was 92.86%;8 cases were cured in the control group1,7 cases were effective,15 cases were ineffective,the improvement rate was 50%;11 cases were cured in the control group2,13 cases were effective,6 cases were ineffective,the improvement rate was 80%.TCMsyndromes integral of the research group before treatment were 17.42 ±6.17,and the scores after treatment are 4.82±1.21.The scores of TCM symptoms before and after treatment were statistically different(p<0.01),which showed that the comprehensive effect of the treatment group was significantly better than that of the control group 1 and 2.4.Comparison of serum levels of human chorionic gonadotropin(beta-HCG)and progesterone(P)before and after treatment in research group:The serum levels of beta-HCG,P in the treatment group were significantly different from those in the control group at the 5th,6th and 8th weeks of gestation(p < 0.05),but there was no significant difference between the serum levels of beta-HCG and P in the 10 th week of gestation and those in the control group 1 and 2(p >0.05).Conclusion: Pretreatment with aspirin and prednisone before pregnancy and sequential treatment with Bushen Huoxue Prescription after pregnancy for RSA of kidney deficiency and blood stasis type can significantly improve the symptoms and pregnancy outcomes of patients with traditional Chinese medicine,and improve the success rate of fetal preservation.Moreover,Bushen Huoxue Prescription can improve the levels of Human Chorionic Gonadotropin(beta-HCG)and Progesterone(P)after pregnancy. |
