Objective: To evaluate the safety of phacoemulsification combined with silicone oil removal and intraocular lens implantation with anterior chamber maintenance.Methods: 80 patients(80 eyes)with concurrent cataract after vitrectomy and silicone oil filling in our hospital from December 2015 to December 2017 were collected and 80 cases(80 eyes)were randomly divided into 40 groups of A and B two groups.The cataract phacoemulsification and the silicone oil removal in the flat ciliary body of the A group were treated with the anterior chamber maintainer perfusion.Implantation of a superimposed intraocular lens(IOL).After phacoemulsification of the anterior chamber maintainer,the patients in the B group underwent the implantation of the foldable intraocular lens(IOL)after the silicone oil removal in the flat part of the ciliary body after the ciliary body perfusion.The time of removal of silicone oil,intraoperative and postoperative complications,ocular surface stimulation after operation,and the best corrected visual acuity after operation were observed in the two groups of A and B,and the time of follow-up was first days,second weeks and third months after operation.The oil taking time,intraoperative and postoperative complications,postoperative ocular irritation and best corrected visual acuity were compared between the two groups.Results: the operations in two groups were successfully completed.The time of taking oil in group A was 4.65 + 0.54 min,and the time of taking oil in group B was 4.28 + 1.62 min.Intraoperative and postoperative complications: 2 cases of posterior capsule rupture in group A,postoperative corneal edema in 3 cases,secondary glaucoma in 2 cases,recurrent retinal detachment in 1 cases,and posterior capsule opacification in 4 cases.In group B,there were 1 cases of posterior capsule rupture,4 cases of corneal edema,1cases of secondary glaucoma,2 cases of recurrent retinal detachment and 2cases of posterior capsule opacification.All patients did not see corneal endothelial decompensation,residual silicone oil bubble,choroidal detachment,intraocular lens displacement,and vitreous hemorrhage after operation.Postoperative ocular surface stimulation symptoms were: postoperative ocular surface stimulation symptoms: postoperative On the first day,group A: 19.63 +2.14,group B 24.24 + 1.17,group A after second weeks of operation,16.51 +3.51,group B: 20.44 + 2.74;third months after operation,group A: 13.16 + 3.15,group B 15.72 + 3.87.The best corrected visual acuity(A)in third months after operation was 0.48 + 0.31 in group B,and 0.43 + 0.29 in group B.Conclusions: cataract phacoemulsification with anterior chamber maintainer perfusion combined with silicone oil removal and intraocular lens implantation in the flat part of ciliary body is a safe and effective combined operation. |
