Objectives:1、Clinical and pathological factors affecting the status of sentinel node(NLSN)metastasis in patients with early breast cancer with positive sentinel node(SLN)metastasis were retrospectively analyzed.2、Establish the regression model of relative risk prediction.Methods:The clinical and pathological data of 64 patients with early breast cancer who presented negative clinical manifestations in the axillary region from June 2014 to September 2018 in the department of general surgery of the First affiliated hospital of Nanchang university were retrospectively collected,who successfully underwent SLN biopsy with positive pathological results and subsequently underwent ipsilateral axillary lymph node dissection,statistical software was used to analyze the relevant factors.The factors with statistical significance in univariate analysis were included in logistic multivariate regression analysis,and the regression model of risk prediction was established by using the factors with statistical significance in multivariate analysis.The validity of the model was tested by using receiver operating characteristic curve(ROC)and hosmer-lemeshow to test goodness of fit.Results:Among the 64 patients collected,a total of 29 cases had positive pathological results of NSLN and 35 cases had negative results.Univariate analysis showed that the metastasis of NSLN was related to the size of the primary tumor(P=0.008),lymphatic vascular invasion(P=0.011),the location of the primary tumor(P=0.011),the number of sentinel lymph node positive(P=0.001),the number of sentinel lymph node negative(P=0.002),and the ratio of positive SLN number to total SLN(P=0.000).Multivariate logistic regression analysis showed that the primary tumor site(P=0.009)and the ratio of positive SLN to total SLN(P=0.001)were independent risk factors for NSLM metastasis.ROC was drawn and the area under the curve(AUC)was calculated to be 0.843 95%CI(0.748-0.939),and the chi-square value of hosmer-lemeshow test for goodness of fit was 3.102 and P=0.541,indicating that there was no significant statistical difference between the predicted value and the actual value,indicating that the model had a good prediction ability Conclusion:In early breast cancer patients with SLN metastasis,the metastasis status of NSLN is affected by the original tumor site and the proportion of positive SLN number to the total number of SLN,and the primary tumor is located in the outer upper quadrant.The greater the proportion of positive SLN in the total number of SLN,the greater the possibility of tumor metastasis of NSLN... |
