Retrospective Study On The Individual Treatment Of Vancouver B2.B3 Fractures With Long Stem Prosthesis Revision Combined With Cable Ring Ligation

Objective: The clinical effects of individualized treatment of VancouverB2 and B3 fractures with long-handled prosthesis revision,wire rope ligation and oral Jiegu Qili capsules were analyzed retrospectively.Methods: A retrospective study was conducted on patients with periprosthetic femoral fractures after hip replacement who were treated in the Affiliated Hospital of Changchun University of Traditional Chinese Medicine and the First Hospital of Jilin University from August 2016 to September 2018.Patients with VencouverB2 and B3 fractures undergoing surgical treatment were individually treated with long-handled prosthesis revision,wire rope ligature and oral Jiegu Qili capsule after operation,and the postoperative curative effect was analyzed and studied.The fracture healing time,hip joint function score and VAS pain score 1 month,3 months and 6 months after operation were observed.Results: Follow-up time ranged from 6 to 15 months,with an average of 9.24 months.Fracture healing time was 8.86±2.03 months.All patients’ fractures healed with good alignment and alignment,and no prosthesis loosening,internal fixation fracture and displacement.Harris score of hip joint was 78.9 ~ 91.7 points,with an average of 84.9points,and the excellent and good rate was 80.9%.The visual analogue scale(VAS)of pain in fracture site was 1-3,with an average of 1.95.Conclusion: The long-handled prosthesis revision combined with steel cable encircling and postoperative oral administration of Jiegu Qili capsule and other individualized treatments for femoral periprosthetic fractures have definite curative effects.During follow-up,the fractures of the patients all achieve healing,the fracture alignment and alignment are good,and there is no prosthesis loosening,internal fixation fracture and fracture displacement.Jiegu Qili capsule can obviously promote fracture healing,reduce fracture non-healing rate,restore joint function and improve the quality of life of the patients.At the same time,Vencouver classification can well evaluate thefracture position,the quality of the bone around the prosthesis,the prosthesis and the fracture stability,and has guiding significance for the treatment of the fracture around the prosthesis after hip replacement.
