Objective:To explore the distribution of TCM syndromes in patients with chronic liver failure and observe the effect of vitamin K1 on survival,liver function and coagulation function of patients with chronic liver failure.Methods:A single-center retrospective cohort study was conducted to collect 90patients with chronic liver failure diagnosed from December 2013 to August 2017 through the electronic case system of the Affiliated Hospital of Changchun University of Traditional Chinese Medicine.The survival time,TCM syndromes,prothrombin time(PT),prothrombin activity(PTA),international standardized ratio(INR)and Child-Turcotte-Pugh(CTP)scores of 90 patients were collected and collated.One is to analyze the distribution regularity of TCM syndromes of chronic liver failure.The other is to compare the therapeutic interventions into four groups:basic treatment+vitamin K1 group,basic treatment+vitamin K1-plasma group,basic treatment+plasma group and basic treatment group.The effects of vitamin K1 and its course on survival,liver function and coagulation function of patients with chronic liver failure were analyzed.Data were analyzed by SPSS22.0 and GraphPad Prism 8 statistical software.P<0.05 had statistical significance.Results:1.The TCM syndromes of chronic liver failure include liver and spleen blood stasis syndrome(36 cases,40.00%),damp-heat accumulation syndrome(28 cases,31.11%),qi stagnation and dampness stagnation syndrome(7 cases,7.78%),stagnation of Liver Collaterals syndrome(6 cases,6.67%),deficiency of liver and kidney Yin Syndrome(3cases,3.33%),Qi deficiency and blood intake syndrome(3 cases,3.33%),stagnation of liver-qi and blood stasis syndrome(2.22%),Qi deficiency and blood stasis syndrome(1Cases,1.11%.Yin deficiency with blood stasis(1 case,1.11%).2.PT(P=0.032),PTA(P=0.028),INR(P=0.036),CTP(P=0.012)were compared before and after treatment in the basic treatment+vitamin K1 group.PT(P=0.000),PTA(P=0.000),INR(P=0.000),CTP(P=0.000)were compared before and after treatment in the basic treatment+vitamin K1-plasma group.PT(P=0.362),PTA(P=0.239),INR(P=0.378)and CTP(P=0.140)were compared before and after treatment in the basic treatment+plasma group.Conclusion:1.Chronic liver failure patients have 10 TCM syndromes:liver and spleen blood stasis syndrome,damp-heat accumulation syndrome,Qi stagnation and dampness obstruction syndrome,blood stasis and Liver Collaterals syndrome,liver and kidney yin deficiency syndrome,Qi insufficiency and blood stasis syndrome,liver and Qi stagnation and blood stasis syndrome,Qi deficiency and blood stasis syndrome and yin deficiency and blood stasis syndrome.2.Vitamin K1 and/or plasma are beneficial for prolonging the survival of patients with chronic liver failure.3.The recommended application time of vitamin K1 for chronic liver failure is 5-10 days.
