| Object:By comparing the clinical outcome of acute type A aortic dissection(ASTAAD)patients underwent total arch replacement with three different surgical techniques,we aim to discuss the best surgical management for ASTAAD patients and guide future clinic practice.Methods:Retrospectively analyzing 133 cases of patients with ASTAAD underwent total arch replacement with three different techniques from February 2015 to September 2018,we divided those patients into Sun’s group,aortic-clamp group and modified island technique group according to the surgical technique employed during the operation and compared the clinical results,including preoperative general condition,cardiopulmonary bypass time,aortic cross-clamp time,postoperative neurological deficit,postoperative transfusion,reoperation,ICU stay,time of mechanical ventilation,continuous renal replacement therapy and so on.Results:There is no statistic difference in preoperative condition,like age,sex and medical history,among three groups.Significant statistic differences were found in operation time,cardiopulmonary bypass time,transient neurological deficit among those groups,while no statistic difference in aortic cross-clamp time,time of mechanical ventilation,mortality,Cre and so on.Conclusion:Both aortic-clamp and modified island technique are safe and effective approach for ASTAAD that can significantly reduce postoperative complication and provide with satisfactory clinical outcome. |
