| ObjectiveTo understand the epidemic characteristics of middle school students’childhood abuse,analyze the relationship between childhood abuse and IL-6 levels of middle school students,and provide evidence-based basis for promoting the physical and mental health of middle school students.MethodsFrom December 2017 to January 2018,a group sampling method was adopted to select 1 086 students from a boarding school in Shenyang as the research object.The Childhood Abuse Questionnaire(CTQ)which has been converted into Chinese was used to understand the abuse experience of the subjects before the middle school,data in the questionnaires that corrected the obvious logic errors and the missing filling rate of 5%or more,and a total of 1 062 valid questionnaires were collected.The level of IL-6 was measured by fasting venous blood of middle school students from 7 to 9 in the morning,the individuals with IL-6 levels greater than the clinical inflammatory disease levels were excluded,and 911 samples with matching questionnaire data and blood data were finally obtained.The database was builtand enteredby using EpiData 3.02,and the data was analyzed by using SPSS 17.0.The distribution of childhood abuse in different demographic characteristics and the difference of IL-6 levels of different grades and gender were analyzed by using chi-square test.Control related extraneous factors,establish univariate logistic regression analysis model,and analyze the impact of childhood abuse on IL-6 levels in middle school students.The study received informed consent from the school,studets and parents.ResultsIn this study,the rate of physical abuse reported that boys(28.4%)was higher than girls(14.6%),Junior school students(29.3%)were higher than senior school students(17.8%),poor academic performance group(21.7%)was higher than moderate group(18.4%)and good(21.7%)group,Junior school boys(40.6%)higher than junior school girls(16.6%)and senior school boys and girls(21.7%,13.5%),the differences were statistically significant(χ~2=25.37,16.07,10.99,45.86;P values were<0.01).The rate of emotional abuse that non-only children(23.5%)were higher than the only children(18.3%),and the family economic conditions were poor group(35.1%),which was higher than the family economic conditions moderate group(20.0%)and good group(16.7%).Poor academic performance group(26.8%)was higher than those with moderate academic performance group(18.4%)and good performance group(17.5%),and the differences were statistically significant(χ~2=3.63,9.12,7.89;P values were<0.05).Sexual abuse reporting rate boys(12.7%)were higher than girls(6%),the family economic conditions were poor group(19.3%)higher than the family economic conditions moderate group(8.2%)and good group(11.9%),junior school boys(13.5%)were higher than junior school girls(7.3%)and senior school girls(5.3%),the differences were statistically significant(χ~2=11.86,8.88,12.49;P values were<0.05).Total abuse reporting rate boys(23.6%)were higher than girls(18.1%),family economic conditions were poor group(40.4%)higher than family economic conditions moderate group(19%)and good group(22.6%),poor academic performance group(29.8%)higher than moderate group(17.2%)and good group(20.1%),junior school boys(27.6%)were higher than junior school girls(20.5%)and senior school boys and girls(21.4%,16.7%).the differences were statistically significant(χ~2=4.20,14.89,15.10,7.67;P values are all<0.05).The rate of higher IL-6 level was higher in overweight/obesity group(20.1%)than normal body weight group(13.1%),poor family economic conditions group(24.6%)were higher than moderate group(12.8%)and good group(16.1%),the differences were statistically significant(χ~2=4.84,6.58;P values were all<0.05).Middle school student abuse was not associated with IL-6 in the total sample.Stratified by gread analysis showed that IL6 in junior school students suffered from high levels of physical abuse(20.0%)and total abuse group(23.1%)was higher than the relevant low level abuse group(11.0%,10.7%),(χ~2=4.76,7.65;P values were all<0.05);Stratify by gender analysis showed that there was no significant difference in abuse and IL-6between different gender groups;the results of the grade-by-gender cross-group analysis showed that IL-6 in junior school boys group had high level physical abuse(24.6%)and emotional abuse(34.5%)and total abuse(31.9%)were higher than the relevant low level abuse group(12.9%,14.5%,12.2%),(χ~2=3.88,6.78,9.10;P values were all<0.05).Establish univariate logistic regression analysis model that control confounding factors,which show that physical abuse(OR=2.23,95%CI:1.02-4.98),emotional abuse(OR=3.49,95%CI:1.36-8.98)and total abuse(OR=1.58,95%CI:1.04-2.41)were independent risk factors for elevated IL-6 levels in junior school boys(P values were all<0.05).ConclusionsChildhood physical abuse,emotional abuse and total abuse could increase of higer IL-6 levels,especially in junior school boys.The abnormal level of inflammatory factors partly explains the adverse health effects of childhood abuse in junior school boys. |
