BackgroundTo investigate the correlation between MR imaging features and Ki67、GPC-3 expression in hepatocellular carcinoma.Mathods53 patients with hepatocellular carcinoma who were confirmed by surgery and pathology were retrospectively analyzed.The patients were divided into two groups by whether the tumors recurred or not,and then the threshold of Ki67 was obtained.The validity of Ki67 was verified by the survival time of the patients.With this threshold,the patients were divided into two groups again,and the relationship between image signs and Ki67 was evaluated.According to GPC-3 index,the patients were divided into two groups: GPC-3 positive group and GPC-3 negative group.The following MRI imaging parameters were analyzed qualitatively:(1)regular or irregular tumor margin;(2)peritumoral enhancement at arterial phase;(3)arterial rim enhancement;(4)internal necrotic or cystic portion;(5)hemorrhage;(6)tumor capsule;(7)satellite nodule;(8)macrovascular invasion.Chi-square test was used to compare the MRI imaging features between the two groups.Results18 patients showed irregular tumor margin,of which 14 were Ki-67 positive(P=0.009),15 were GPC-3 positive(P=0.023).According to the degree of enhancement in the arterial phase,16 patients showed circular enhancement in the arterial phase,of which 13 were positive for Ki-67 index(P=0.006),15 were GPC-3 positive(P=0.002).A total of 11 patients with macrovascular invasion,of which 9 were Ki-67 positive,including hepatic artery,hepatic vein,portal vein and vena cava(P=0.031).There were 8 cases of intratumoral hemorrhage,and their expression of GPC-3 were positive(P=0.046).ConclusionIn the qualitative analysis of MR images,irregular tumor margin and arterial rim enhancement are important for predicting the positive expression of Ki-67 and GPC-3.Macrovascular invasion can predict the expression of Ki-67 to some extent,while intratumoral hemorrhage can predict the positive expression of GPC-3.These imaging findings provides imaging basis for understanding the biological behavior of hepatocellular carcinoma,evaluating the prognosis of patients,and the choice of clinical treatment.
