| Objective:To investigate the role of nanocarbon suspensions in the identification and protection of parathyroid glands in extended radical operation of thyroid cancer,whether it can reduce the incidence of temporary and permanent hypoparathyroidism and whether it can improve the thoroughness of central lymph node dissection and the detection rate of metastatic lymph nodes.Methods:Retrospective analysis of 241 patients who underwent primary surgery for thyroid surgery at the First Hospital of Jilin University from January 2015 to December 2017.All surgical procedures were thyroidectomy plus bilateral central lymph node dissection and postoperative pathology confirmed papillary thyroid carcinoma.According to whether the patients use nanocarbon in the operation,they are divided into experimental group and routine operation group,there were 122 cases in the nanocarbon experimental group,16 male patients,106 female patients,with an average age of 43.88±8.80 years;there were119 cases in the routine operation group,16 male patients,103 female patients,with an average age of 43.23±9.25 years.The blood calcium levels,parathyroid hormone levels were recorded before and 1 day,1month,3 months,and 6 months after surgery.The number of central lymph node dissection,central lymph node metastasis,the number of parathyroid miscuts and the number of cases of permanent hypoparathyroidism were recorded after surgery.Results:There were no statistical differences in age,gender,preoperative blood calcium and preoperative parathyroid hormone levels between the two groups(P>0.05),indicating that the two groups were comparable.The incidence of postoperative temporary hypocalcemia was 36.89%(45/122)in the nanocarbon experimental group,and the incidence of temporary hypocalcemia was 53.78%(64/119)in the routine operation group,the difference between the two groups was statistically significant(P<0.05);the incidence of temporary hypoparathyroidism in the nano-carbon experimental group was 32.79%(40/122),and the incidence of temporary hypoparathyroidism in the routine operation group was47.06%.56/119),the difference between the two groups was statistically significant(P<0.05);2 patients in the nanocarbon experimental group had parathyroid missection,8 patients in the routine operation group had parathyroid missection,the difference between the two groups was no statistically significant(P>0.05);One case of permanent hypoparathyroidism occurred in the nanocarbon experimental group after operation,four cases of permanent hypoparathyroidism occurred in the routine operation group,there was no statistical difference between thetwo groups(P>0.05).In the nanocarbon experimental group,1110 central lymph nodes were detected,with an average of 9.10±5.17.In the routine operation group,928 central lymph nodes were detected,with an average of 7.80±4.73,the difference was statistically significant(P<0.05);169metastatic lymph nodes were detected in the nanocarbon experimental group and 127 metastatic lymph nodes were detected in the routine operation group;the difference between the two groups was not statistically significant.Conclusion:Application of nano-carbon suspension in extended radical operation of thyroid cancer can reduce the incidence of postoperative temporary hypocalcemia and reduce the incidence of postoperative temporary hypoparathyroidism rate and has a protective effect on the parathyroid gland;the application of nano-carbon suspension in the extended radical operation of thyroid cancer can increase the thoroughness of lymph node dissection in the central region of the neck,but can not improve the detection rate of metastatic lymph nodes. |
