| Objective:Two-dimensional ultrasound,color Doppler flow imaging(CDFI),pulsed Doppler ultrasound(PW)were used to detect the kidney of patients with gout,and ultrasonic classification of kidney stones was performed.To investigate the discrepancies between ultrasound classification of kidney stones and blood uric acid level,course of disease,renal blood flow parameters,patient age,BMI and gender in patients with gout.Methods: From February 2017 to August 2018,collecting 80 patients with gout diagnosed in the First Affiliated Hospital of Guangxi Medical University.Two-dimensional and Doppler ultrasonography was performed on a total of 160 kidneys in 80 patients.Two-dimensional ultrasound observed the condition of renal calculi,CDFI showed renal artery blood flow filling,PW measured renal artery blood flow parameters.According to the occurrence of kidney stones by two-dimensional ultrasonic detecting,the bilateral kidneys are divided into ultrasound type I(no stone type),type II(kidney and pyelone calculus),type III(kidney cone echo enhanced type),and patients with gout are classified into group A(both kidneys are type I),group B(one or both kidneys are type II),and group C(one or both kidneys are type III),according to the typeof kidney stone.The differences in blood uric acid value,duration of disease,patient age and BMI were compared between the group A,B,and C.And comparing differences in PSV,EDV,and RI of renal segmental arteries,interlobular arteries,and interlobular arteries between type I,II,and III.Results: 1、The uric acid level in group C was higher than that in group B and group A,and group B was higher than group A.There were significant differences in serum uric acid value between the three groups(P<0.05).2、The patients in group C were longer than group B and group A,and group B was longer than group A.There were significant differences in the course of disease between the three groups(P<0.05).3、The PSV and EDV of renal segmental artery,interlobar artery and interlobular artery in type III were lower than those in type I and II,and the RI was higher than type I and type II(P<0.05).There were significant differences in PSV,EDV and RI between type I,II and III renal segmental arteries,interlobular arteries and interlobular arteries(P<0.05).4、There was no statistically significant difference in patient age,BMI and gender between the three groups(P>0.05).Conclusions: Ultrasound diagnosis of renal stones in patients with gout and their classification can be used to judge the severity of kidney stones,and to some extent reflect the progression of patients with gout.The renal arteries of the type III kidney has a lower blood flow velocity and a higher resistance,which can provide reference for clinicians to choose more suitable treatment plans and therapeutic drugs. |
