Objective:To analysis of the clinical features,treatment and prognosis of post-stroke epilepsy.Methods:221 patients with post-stroke epilepsy admitted to our hospital from January2009 to December 2017 were included in our study.The clinical datas,such as general condition,stroke type,seizure time and so on,were studied by means of a retrospective analysis.Results:Among the 221 patients,161 were males and 60 were females.They were between 30 and 87 years old,with the highest incidence among middle-aged and elderly people under 70 years old.Most patients have risk factors for cerebrovascular disease such as stroke and hypertension before onset.38.9% of patients had ES,61.1%of patients had LS,and cerebral hemorrhage and cerebral infarction were tend to cause LS.73.3% of ES patients appeared within 24 hours after the onset of stroke,and49.6% of LS patients appeared 1 year after the onset of stroke.PSE patients with partial seizures followed by full seizures were the most common,accounting for56.9%;complex partial seizures accounted for 13.3%;simple partial seizures and generalized tonic-clonic seizures each accounted for 12.7%;status epilepticus accounted for 4.4%.In the single lesion patients,87.8% of them affected cortex,including the frontal lobe(14.5%)and temporal lobe(9.9%).EEG was performed in181 patients,and 86.2% of them were abnormal,which mostly observed epileptic discharge waves such as sharp waves,spikes,sharp waves,and spine slow waves.The difference between abnormal EEG and normal EEG in recurrence rate was statistically significant(P=0.016,<0.05).170 patients were treated with drugs,and94.1% of the patients were treated with monotherapy to control symptoms.93.2% of patients had improvement,35.0% of patients had recurrence,and the difference between cerebral hemorrhage and cerebral infarction in recurrence rate was statistically significant(P=0.035<0.05).Complications occurred in 32.1% of patients,including pneumonia(33.0%)and respiratory failure(6.8%).There was a statistically significant difference in the incidence of complications between patients with early onset and late onset epilepsy(P=0.0005 <0.05).Conclusions:74.2% of patients with PSE were found in middle-aged and elderly people under70 years old,and about 85.5% of patients had a history of stroke before onset.61.1% of patients were occurred LS,and 73.3% of ES patients occurred within 24 hours after the onset of stroke.82.9% of patients with PSE were partial seizures.The partial seizures followed by full seizures of ES and LS patients accounted for 50.7%and 60.9%,respectively.93.2% of patients had improvement,and 35.0% of patients had recurrence.Patients with cerebral hemorrhage,late onset epilepsy,and abnormal EEG were risk factors for recurrence of epilepsy.
