Clinical Efficacy Of TKA On Knee Osteoarthritis With Moderate And Severe Varus Deformity

Objective:To analyze the clinical effect of primary TKA on knee osteoarthritis with moderate and severe varus deformity.To study the causes of varus Angle of knee joint in patients with severe varus deformity and the selection of surgical scheme and operation skills.Methods:Retrospective analysis was made on the clinical data of 50 patients who received total knee arthroplasty for correcting moderate and severe knee varus deformity in(>10°)[1]the implementation of total knee arthroplasty patients with knee osteoarthritis treatment,Fifty patients who met the inclusion criteria of our study were selected and divided into two groups according to the Angle of genu varus:moderate varus and severe varus moderate varus 26 patients.24 cases of severe varus.The surgery was performed by the same operator and team,during which osteophytes were cleaned up,soft tissues were loosened.bone defects were treated,and lower limb line was restored to correct varus deformity.Routine antibiotics were used to prevent infection,anticoagulants and air pressure pumps were used to prevent thrombosis and under the guidance of doctors for rehabilitation.Preoperative and postoperative femoral tibial Angle,articular Angle,tibial Angle and tibial varus Angle were measured,and HSS scores were used for regular follow-up.Results:the mean follow-up time was 18.8 months(12to30 months).No case was lost.The operation time was 58～94 min,with an average of 73 min.Preoperative measuring average tibiofemoral Angle of knee varus Angle for 13.1°,which soft tissue varus(58%),bone structural varus accounted for 38%,mixed 4%.After TKA surgery average tibiofemoral Angle of knee valgus Angle is 5.3°,the average correct Angle is 18.4°(soft tissue balance correction(64.1%),cut bone correction(35.9%).No complications such as vascular and nerve injury,infection and patellar fracture occurred,and the situation of varus knee was significantly improved compared with that before operation.During the follow-up,X-ray images were reviewed,the prosthesis was in good position,and no prosthesis loosening or subsidence occurred.Average preoperative HSS score of knee joint(38.24±3.37).HSS score(85.62±6.32)in the knee during the final follow-up was significantly better than that before surgeryConclusion:TKA treatment of knee osteoarthritis with moderate and severe varus can effectively correct varus deformity and restore lower limb force line,and knee joint activity and function is satisfactory.The analysis of the causes of knee varus before operation is helpful for the correct treatment of soft tissue balance,osteotomy and bone defect.There was no significant difference in postoperative efficacy between the moderate varus group and the severe varus group.Patients with severe varus deformity still need to pay attention to the importance of soft tissue balance.
