Objective By immunohistochemical staining method to detect lesion with psoriasis vulgaris in CXC chemokines family CXCL1,CXCL5,CXCL10,CXCL16 CXCR1 and receptors,CXCR2,CXCR3,CXCR6 expression,to explore the pathogenesis of psoriasis,and analysis of tripterygium wilfordii glycosides was detected after treatment in CXC chemokines and their receptors expression family conditions change,explore the related mechanism of tripterygium wilfordii more glycosides treatment of psoriasis.Methods Forty patients with psoriasis vulgaris who met the diagnostic criteria were collected.Oral administration of tripterygium wilfordii polyglycoside tablets for treatment.Blood routine tests and liver and kidney function tests were performed in all patients before treatment and at weeks 4 and 8 after oral administration of tripterygium wilfordii polyglycosides.The same patient underwent histopathological examination before(during)treatment and after(during)treatment,and the patients with skin lesions before and after treatment were excluded.A total of 15 cases and 30 specimens were collected,and 5 tissue specimens of the normal control group were obtained from the normal tissues of 5 plastic surgery patients.According to the psoriasis area and severity index(PASI)scoring criteria,PASI scores were given to each patient before treatment and 8 weeks after treatment respectively,and the scores,adverse reactions during the administration of triplet glycosides and other conditions were recorded in detail.The skin tissues of the experimental group and the control group were observed by immunohistochemistry,and micrograph was taken.SPSS22.0for Windows software was used for statistical analysis of the data,t test was used for PASI score comparison,and anova was used for mean comparison of multiple samples of immunohistochemical results.P<0.05 was considered statistically significant.Results ①Clinical efficacy:all 40 patients with psoriasis vulgaris completed 8 weeks of treatment,and there was a significant difference in PASI score before and after treatment(t=13.71,P<0.01).②In the study of triptetygium wilfordii)the incidence of adverse reactions was 10.00%(4/40),mainly for nausea,anorexia,loose stool and defecate number increase,all of the patients before and after treatment and during treatment were performed routine blood and liver and kidney function monitoring,which appear in 1 patients elevated alanine aminotransferase(ALT)transient,1 cases of patients with glutamic acid transaminase(AST)transient rise,2 cases of female patients with temporary menstrual quantity decreases.3.The expression level of CXCL1 in psoriasis progressive skin lesions was significantly increased in the stratum spinosum.After treatment with tripterygium wilfordii,the expression level in the stratum spinosum decreased to be similar to that of the normal control group.The expression level of CXCL5 in psoriatic lesions at progressive stage and degenerative stage was significantly higher than that in normal control group,and the expression pattern of CXCL5 in keratinocytes in psoriatic lesions at progressive stage was different from that in normal group and degenerative stage.The expression level of CXCL10 in psoriatic lesions of the basal layer decreased and increased gradually to the granular layer.The expression level in the granular layer was significantly higher than that in the normal control group,while the expression level was negative in the cuticle.The expression of CXCL16 in the superficial layer of the epidermis was higher than that in the subepidermis,while the expression of CXCL16 in the subepidermis and capillary endothelial cells in psoriatic lesions was significantly higher than that in the normal skin,while no expression of CXCL16 was observed in inflammatory cells in the dermis.There were no statistically significant differences in the staining of various layers of skin damaged keratinocytes in psoriasis,CXCR1,CXCR2,CXCR3 and CXCR6 receptors(P>0.05).Conclusion CXCL1,5,10,16 common expression within the lesion with psoriasis vulgaris,may be in different degree,different ways are involved in the pathogenesis of with psoriasis vulgaris,tripterygium wilfordii glycosides tablets in effectively improve the detected at the same time,can change within the lesion of chemokines CXCL1,5,10,16 expression level and the expression pattern,therefore,change the expression of chemokine levels and/or expression patterns may be looking for the breakthrough point of the new method for the treatment of psoriasis.CXCR1,2,3,6 in skin lesions and normal skin with psoriasis vulgaris common expression,the expression mode and strength did not see obvious difference,may not play a critical role in the pathogenesis of psoriasis,but because of its corresponding abnormal expression of chemokines,such receptors are still likely to be targets for the treatment of psoriasis,deserves further research. |
