Hypnotic Drug Use and its Influencing Factors with Chronic InsomniaObjective In order to improve the clinical standardization and rational use of hypnotic drugs,improve the physician’s indications of hypnotizing drugs and contraindications,the current status of the use of hypnotic drugs and the influencing factors affecting the use can be well known and promote the sedative and hypnotic drugs.Clinically,it can be used reasonably and allows patients to obtain a more rational treatment plan.Method Randomly selected from January 2016 to May 2017,2 371 patients with chronic insomnia at the Sleep Disorders Clinic in Hangzhou completed general information and scale assessment.The questionnaire included the Pittsburgh Sleep Quality Index Questionnaire(PSQI)and the Broad Anxiety Scale(GAD-7).Using SPSS17.0 software for analysis,in the study of the influencing factors of insomnia,the chi-square test was used to compare the differences in gender and family history between different levels of insomnia group.One-way analysis of variance was used to compare the age differences among patients with different degrees of insomnia.The rank sum test was used to compare the differences in disease duration and education level among different insomnia groups.For the factors of P<0.01 in the above analysis,the multivariate unconditional logistic regression model was selected to continue to include the influencing factors for further analysis of insomnia.P<0.05 was statistically significant.significance.Results Of the 2371 patients surveyed,1512(63.77%)were women,859(36.23%)were male,the average age of women was(40.17±11.78)years old,and the average age of men was(38.76±11.55)years;patients who used hypnotic drugs 1082 persons(45.63%),mainly benzodiazepines(42.05%),non-benzodiazepines(21.35%)and hypnotic antidepressants(19.22%);gender,age,education There was a statistically significant difference in the use of hypnotic drugs between chronic insomnia patients with degree,duration of disease and severity of insomnia(P<0.05).Women were more likely to use hypnotic drugs than men(P<0.05).The older the age,the lower the education level,the insomnia The longer the course of disease and the more severe the insomnia,the more likely the hypnotic drugs were used(P<0.05).Multivariate analysis showed that age,duration of disease and insomnia were the main influencing factors of hypnotic drugs in patients with chronic insomnia(P>0.05).Conclusion This study shows that women,older,less educated,longer insomnia,and more severe insomnia,the frequency of hypnotic use is often higher.1.Among patients with chronic insomnia,women are more likely to use hypnotic drugs than men.2.Among patients with chronic insomnia,older patients are more likely to use hypnotic drugs.3.In patients with chronic insomnia,insomnia patients with a disease course of 1 year to 3 years are most likely to use hypnotic drugs.4.Among patients with chronic insomnia,patients with severe insomnia are more likely to use hypnotic drugs than patients with mild insomnia.Therapeutic Effect of Network-Based Insomnia Cognitive Behavior Therapy Combined with Hypnotic Drugs on Patients with Chronic InsomniaObjective In order to improve the clinical standardization and rational use of cognitive behavioral therapy,the promotion of network-based insomnia cognitive behavior therapy to better serve insomnia patients and improve clinical utilization.Exploring the treatment of chronic insomnia,optimizing the efficacy,so that patients with insomnia get more professional and standardized diagnosis and treatment.Method A total of 768 patients with chronic insomnia who were admitted to the Sleep Disorders Clinic in Hangzhou from December 2016 to April 2017 were randomly selected for general information and scale assessment.The questionnaire included the Pittsburgh Sleep Quality Index Questionnaire(PSQI),the Broad Anxiety Scale(GAD-7),and the Depression Screening Scale(PHQ-9).Network-based insomnia cognitive treatment group(Network-based insomnia cognitive treatment group(NCBT-I)and network-based insomnia cognitive therapy combined with network-based insomnia cognitive therapy combined with network analysis-based analysis of variance analysis Hypnotic drug group(hereinafter referred to as NCBT-I+ Drug),the degree of insomnia,anxiety and depression in chronic insomnia between the two groups.The analysis was performed using SPSS 17.0 software.In the NCBT-I group and the NCBT-I+ Drug group 2,the gender difference in the categorical variables at the baseline assessment was compared using the chi-square test method to compare the gender differences between the two groups.The age of continuous variables was compared by one-way analysis of variance for the age difference between the two groups.For the comparison of the education level of the grade factors,the rank sum test was used to compare the education levels of the two groups.For the comparison of therapeutic effects,repeated measures ANOVA were used to compare the baseline,2 weeks,4 weeks,6 weeks,and 8 weeks of PSQI,GAD7,and PHQ9,and the efficacy of the two groups of insomnia patients was evaluated.Result Among the 768 patients surveyed,there were 485 in the NCBT-i group,including 330 females and 155 males,and 283 in the NCBT-I+ Drug group,including 208 females and 75 males.The age ranged from 18 to 65 years old,the average age of the NCBT-I group was(45.87±11.04)years old,and the average age of the NCBT-I+ Drug group was(43.54±10.73)years old.The NCBT-I group and the NCBT-I+ Drug group had statistically significant changes in insomnia(F=184.34,P<0.05).There was a statistically significant difference in the treatment of chronic insomnia between the NCBT-I+ Drug group and the NCBT-I+ Drug group(F=4.22,P<0.05).Compared with the NCBT-I group,the degree of insomnia in the NCBT-I+ Drug group was significantly better than that in the NCBT-I group.There was an interaction between the follow-up time and the severity of insomnia(F=4.38,P<0.05).Both NCBT-I group and NCBT-I+ Drug group were effective for anxiety and depression(P<0.05).Conclusion In patients with chronic insomnia,network-based insomnia cognitive behavioral therapy and hypnotic drugs can improve the symptoms of insomnia,anxiety and depression in patients with chronic insomnia.1.Among patients with chronic insomnia,cognitive behavioral therapy group and cognitive behavioral therapy combined with hypnotic drug group have significant improvement in insomnia.2.Compared with the cognitive treatment group,the cognitive behavioral therapy combined with the hypnotic group improved the insomnia more significantly.3.Among patients with chronic insomnia,cognitive behavioral therapy combined with hypnotic therapy group had a faster onset than cognitive behavioral therapy alone.4.Cognitive behavioral therapy group and cognitive behavioral therapy combined with hypnotic drug group were effective in improving anxiety and depression in patients with chronic insomnia.5.Cognitive behavioral therapy group and cognitive behavioral therapy combined with hypnotic drug group were effective for anxiety and depression in patients with chronic insomnia,but there was no significant difference. |
