Objective:Stroke is a common disease that seriously endangers the health of the elderly and is the third leading cause of death in the world.Post-stroke depression(PSD)is a common neuropsychiatric complication associated with stroke.In addition to neurological symptoms,daily life and social impairment,cognitive impairment is more pronounced,especially short-term memory loss,decreased executive decision making and attention disorder.More and more research has been done on PSD in recent years.The prevalence rate is increasing,but neurologists are not aware of it.Therefore,early screening and diagnosis of PSD is particularly important.Event-related potentials P300 mainly reflects the electrophysiological changes in the processing of information such as attention,memory and other information in the cognitive process,especially in the diagnosis of depression.The purpose of this study was to evaluate the cognitive dysfunction and depression state of PSD by using P300 combined with MMSE MoCA,HAMD,and to analyze the relationship between P300 and PSD in order to provide evidence for early diagnosis and treatment in the future.And early intervention treatment was carried out.Methods:From January 2017-February 2018,70 stroke patients were selected from Department of Neurology,Department of Neurology,third people’s Hospital of Bengbu,and all patients were followed up for one year.The diagnosis of cerebral infarction or cerebral hemorrhage by clinical diagnostic criteria(cranial MRI or cephalic CTS)was based on the criteria of depression in CCMD-3.According to whether or not depressive symptoms appeared afterwards,31 patients with PSD were divided into study group,including 14 males.17 women,aged 40-80 years,were educated for 0-15 years.39 patients with post-stroke non-depression NPSD were selected as controls.Group in the control group.20 cases were male,19 female,age 40-80 years;education for 0-15 years.All the researchers or their families were informed consent.All were examined by P300,and MMSE,MoCA were used to evaluate the cognitive function,HAMD scale and severity of depressive mood.Two groups of P300 latency and amplitude and neuropsychological score of clinical data such as data entry,data were analyzed by SPSS17.0 software.Results:1.This study included 70 stroke patients,including 31 patients with PSD,15 patients with mild depression,13 patients with moderate depression and 3 patients with severe depression,with an incidence of 4.3%.There was no significant difference in MMSE score and amplitude between the two groups(P > 0.05).2.Compared with the control group,the lower score of MoCA scale and the higher P300 latency of Hamd scale in the study group were significantly longer than those in the control group(P < 0.05).3.There was a weak negative correlation between P300 latency and MMSE score,a middle negative correlation with MoCA score,and a strong positive correlation between P300 latency and HAMD score.Conclusion:1.The incidence of PSD was 44.28%.2.MoCA scale was more sensitive than MMSE scale in detecting cognitive impairment of PSD patients.3.P300 latency was in good agreement with the results of MMSE MoCA and HAMD scales.P300 combined with psychological scale can more sensitively detect early cognitive dysfunction in patients.To sum up,P300 latency can be used as a biological index for PSD screening,which can be used for early identification and diagnosis.Therefore,early intervention therapy is worthy of further clinical application. |
